摘要
目的:观察胚胎早期卵裂用于预测体外受精-胚胎移植妊娠结局的价值。方法:2009年8月至2011年5月在我院生殖中心首次因输卵管因素行常规体外受精-胚胎移植治疗的136个周期,授精25～27h后观察2PN受精卵是否出现早期卵裂,第3天行胚胎移植时分为移植胚胎中有早期卵裂胚胎组(A组)和无早期卵裂胚胎组(B组)。A组含有1个早期卵裂胚胎的为A1组,有2个早期卵裂胚胎的为A2组,有3个早期卵裂胚胎的为A3组。比较各组之间优胚率、种植率及妊娠率。结果:有早期卵裂的胚胎优胚形成率(63.3%)高于无早期卵裂的胚胎优胚率(49.0%),差异有统计学意义(P<0.01);移植胚胎中有早期卵裂胚胎组(A组)种植率、妊娠率高于无早期卵裂胚胎组(B组)(35.9%vs 22.2%;53.2%vs 33.7%),A1组种植率高于A2组(23.3%vs 45.7%),差异均有统计学意义(P<0.05)。结论:观察胚胎早期卵裂可预测胚胎发育潜能,有利于提高体外受精-胚胎移植种植率。
Objective：To investigate the predicted value of embryo early cleavage for the pregnancy outcome of IVF-ET.Methods：This retrospective study was carried out on 136 infertility cycles with oviduct factors who took the first IVF-ET treatment in our center from Aug.2009 to May 2011.Observed wether early cleavage（EC） occurred after insemination 25~27h.In day 3,it was divided into 2groups：the transfered embryos whose EC were observed（group A）,the transfered embryos whose EC were not observed（group B）.Meanwhile,group A was divided into three subgroups according to the numbers of the transfered embryos whose EC were observed.Al group：one of the EC embryos were observed in ET embryos.A2 group：two of the transfered embryos EC were observed.A3 group：All the three transfered embryos EC were observed.The rate of high quality embryos,embryo implantation,clinical pregnancy were compared.Results：The rate of high quality embryos were significantly higher in the embryos with EC than the embryos with no EC（63.3% vs 49.0%）（P 0.01）,the rate of embryo implantation,clinical pregnancy was significantly higher in the A group than in the B group（35.9% vs 22.2%;53.2% vs 33.7%）.the rate of embryo implantation was significantly higher in the A1 group than in the A2 group（23.3% vs 45.7%）（P 0.05）.Conclusion：The results indicate that embryo early cleavage can predict the potential of embryonic implantation and improve the rate of embryo implantation in IVF-ET.
