摘要
目的掌握湖北省武汉城市与农村15岁及以上成人吸烟现状,为探索城市与农村不同的控烟干预策略提供理论依据。方法采用分层多阶段整群概率抽样法,在武汉市抽取5000名15岁及以上城市和农村居民进行人户问卷调查,根据2015年武汉市普查人口进行加权计算,采用总吸烟率、现在吸烟率、二手烟暴露率等指标对调查结果进行统计分析。结果共回收有效问卷4311份,城市成人现在吸烟率为23.5%,男性现在吸烟率44.1%,有54.1%的吸烟者每天吸烟超过15支;农村成人现在吸烟率为29.6%,男性现在吸烟率为56.9%,有71.8%的吸烟者每天吸烟超过15支;城市和农村居民在室内工作场所中总体二手烟暴露率分别为53.0%和51.6%,在家庭中总体二手烟暴露率分别为40.1%和51.1%,城乡间居民比较差异均无统计学意义(P=0.80,P=0.06)。结论武汉农村成人吸烟率、男性吸烟率、吸烟者的吸烟量及农村家中二手烟暴露均高于城市成人。要制订并运用针对农村成人的综合控烟干预策略,尽快颁布实施《武汉市公共场所控制吸烟条例》,降低吸烟率,减少二手烟的暴露率。
Objective To analyze the smoking prevalence among adults in Wuban City of Hubei Province, and provide a scientific basis for different strategies of tobacco control in urban and rural areas. Methods Stratified cluster ran- dom sampling was used to select 5000 residents aged 15 and older in urban and rural districts of Wuhan City for a face-to- face questionnaire survey. Based on 2015 census population of Wuhan City, all the data was weighted. The smoking status in adults was analyzed by using total smoking rate, current smoking rate, smoking cessation rate and second-hand smoking rate. Results A total of 4311 valid questionnaires were recovered. The current smoking rate and male current smoking rate of urban residents was 23.5% and 44. 1% , respectively. 54. 1% urban residents smoked more than 15 cigarettes per day. The current smoking rate of rural residents and rural male residents was 29. 6% and 56. 9% , respectively. 71.8% rural residents smoked more than 15 cigarettes per day. The second-hand smoking rate of urban and rural residents in indoor work- place was 53.0% and 51.6%, respectively. Compared to urban residents （40. 1% ）, the second-hand smoking rate at home was higher among the rural residents （51.1% ）. There was no statistically significant difference in rate of second-hand smoking among urban and rural residents, either in indoor workplace （ P = 0. 80） or at home （ P = 0. 06 ）. Conclusion Smoking rate, male smoking rate, smoking amount and second-hand smoking rate at home of rural residents in Wuhan were higher than that in urban residents. Comprehensive strategies of intervention on tobacco control need to be developed and ap- plied. To reduce smoking and second-hand smoking, regulations on tobacco control in public place of Wuhan need to be en- acted as soon as possible.
