激光周边虹膜切开术对角膜内皮影响 被引量:5

Effect of laser peripheral iridectomy on corneal endothelial cells

摘要 目的评价青光眼激光周边虹膜切除术对角膜内皮的影响。方法临床病例对照研究。收集2010年1月至2011年12月在深圳市第二人民医院眼科诊断为闭角型青光眼患者125例(167只眼),由同一位医师行激光周边虹膜切开术,术前和术后1周、1个月及3个月采用角膜内皮镜检测角膜上部、中央区和下部的角膜内皮,测量角膜内皮细胞的密度、六边形细胞比率和细胞面积。结果104例(141只眼)完成3个月的随访。术后角膜内皮细胞密度下降、六边形细胞比例下降、细胞面积扩大。角膜内皮的损伤上方最重,中部次之,下部无明显损伤。角膜上部、中央区和下部的角膜内皮细胞密度、六边形细胞比例和角膜内皮细胞面积差异在1周时差别最大,差异有统计学意义(均P〈O.01)。结论激光周边虹膜切开术对角膜内皮细胞损伤小,损伤部位以角膜上部为主。激光周边虹膜切开术是闭角型青光眼一种安全有效地治疗方式。 Objective To investigate the effect of laser peripheral iridectomy on corneal endothe- lial cells. Methods One hundred and twenty-five angle-closure glaucoma patients (167 eyes) were collected from January 2010 to December 2011 in our hospital. Laser peripheral iridectomy was per-formed by the same physician. The corneal endothelial cells on the upper, the central and the lower part of cornea were measured using corneal endothelial microscope preoperative and 1 week, 1 month and 3 months postoperative. The corneal endothelial cells density, hexagonal cell ratio and cell area were calculated. Results Of 104 patients (141 eyes) completed the 3-month follow-up. De-creased corneal endothelial cell density, declined in the proportion of hexagonal cells and the expand-ed cell area were found after operation, The damage of corneal endothelial cells was most severe at the upper part of cornea, following the central part of cornea. The damage of corneal endothelial cells at the lower part of cornea was no statistical significance as compare to pre-operation. The dif-ferences among the upper, the central and the lower part of the corneal endothelial cells density, the hexagonal cell ratio and the cell area were the largest at 1 week after operation and were statistical significances (all P 〈0.01). Conclusions Laser peripheral iridotomy has a small amount of damage on corneal endothelial cells, and the damage position is mainly at the upper part of corneal. Laser peripheral iridotomy is a safe and effective treatment for angle-closure glaucoma.
