摘要
目的观察在可植入接触镜(ICL)植入术时采用不同的虹膜周边切除方式对角膜内皮的影响。方法按虹膜周边切除方式不同将于我院行ICL植入术的病例分为术前激光虹膜周边切除组与术中虹膜周边切除组。角膜内皮镜下观察2组不同时间点角膜内皮情况,并进行统计学分析。结果术前激光虹膜周边切除组行激光术后1周、ICL植入术后1周及1个月时,角膜内皮细胞密度与六边形细胞百分比较术前下降,最大细胞面积与平均细胞面积较术前升高,具有统计学意义;术后3个月时,角膜内皮的各项变化均不具有统计学意义;术中虹膜周边切除组在术后1周、1个月及3个月时,角膜内皮的各项变化均不具有统计学意义;在术后1周时,术中虹膜周边切除组角膜内皮细胞膜密度与六边形细胞百分比较术前激光周边虹膜切除组高,最大细胞面积与平均细胞面积较术前激光周边虹膜切除组小,具有统计学意义。结论术前激光周边虹膜切除后行ICL植入,在早期对角膜内皮细胞有一定的影响,3个月后基本恢复;ICL植入术中行虹膜周边切除对角膜内皮的影响不具有统计学意义。
Objective To observe the influence of different types of peripheral iris resection on corneal endothelial cells in implantable contact lens（ ICL） implantation. Methods Considering different ways of the peripheral iris resection,patients who had undergone implantable contact lens implantation were divided into two groups： laser peripheral iris resection group and intraoperative peripheral iris resection group. The corneal endothelium was observed at different time points by non-contact specular microscopy. Results The cell density and percentage of hexagonal endothelium of laser peripheral iris resection group declined,the maximum cell area and the average cell area increased at 1 week after the laser treatment,1 week and 1 month after operation,and the difference was statistically significant.However,there was no statistically significant difference at 3 months after ICL implantation. In the intraoperative peripheral iris resection group,there was no significant difference on any variations of corneal endothelium at all time points. At 1 week after operation,the cell density and percentage of hexagonal endothelium were higher,the maximum cell area and the average cell area were lower in iris resection group than those in laser peripheral iris resection group,with the difference being statistically significant. Conclusions Preoperative laser peripheral iris resection in ICL implantation had a certain influence on corneal endothelial cells in short time. And the influence disappeared after 3months. There was no influence of peripheral iris resection in ICL implantation on corneal endothelium.
