摘要
目的:探讨超声乳化联合小梁切除术对患者角膜内皮细胞丢失的影响。方法:选取2016年1月-2017年9月本院收治的100例闭角型青光眼合并白内障患者作为研究对象,按照随机数字表法将其分为研究组(n=50)和对照组(n=50)。对照组采用小梁切除术,研究组在对照组的基础上联合应用超声乳化,分析两组患者术前和术后1、3、6个月角膜中央区内皮细胞密度。结果:研究组患者术后1、3、6个月角膜中央区平均内皮细胞密度分别为(1 664±425)、(1 532±414)、(1 527±402)个/mm^2,均明显低于对照组的(1 832±413)、(1 782±411)、(1 739±406)个/mm^2,差异均有统计学意义(P<0.05);研究组患者术后1、3、6个月角膜中央区平均内皮细胞丢失率均明显高于对照组,差异均有统计学意义(P<0.05)。结论:与单纯使用小梁切除术治疗闭角型青光眼合并白内障相比,超声乳化联合小梁切除术治疗方案患者角膜内皮细胞丢失现象更为严重,恢复较慢。术前应检查角膜内皮细胞,严格掌握手术适应证。
Objective：To investigate the effect of phacoemulsification combined with trabeculectomy on corneal endothelial cell loss in patients.Method：A total of 100 patients with angle-closure glaucoma and cataract in our hospital from January 2016 to September 2017 were selected as research objects.According to the random number table method,they were divided into study group（n=50） and control group（n=50）.The control group was treated with trabeculectomy,the study group was treated with phacoemulsification on the basis of the control group.The density of endothelial cell in the central area of cornea in the two groups were analyzed before and 1,3,6 months after operation.Result：The mean endothelial cell density in the central area of cornea in the study group at 1,3 and 6 months after operation were（1 664±425）,（1 532±414） and（1 527±402）case/mm2,which were significantly lower than（1 832±413）,（1 782±411）,（1 739±406）case/mm2 in the control group,the differences were statistically significant（P〈0.05）.The mean loss rate of endothelial cell in the central area of cornea in the study group at 1,3 and 6 months after operation were significantly higher than those in the control group,the differences were statistically significant（P〈0.05）.Conclusion：Compared with the simple trabeculectomy,the corneal endothelial cell loss in phacoemulsification combined with trabeculectomy is more serious and the recovery is slower for angle-closure glaucoma combined with cataract.Corneal endothelial cells should be checked before operation to strictly control the indications for operation.
