目的总结并分析海南省近10年发生的常见过敏性疑似预防接种异常反应(adverse event following immunization,AEFI)案例图谱,摸索出切实有效的临床实用处理办法,为妥善处理过敏性异常反应积累案例经验。方法各级预防接种单位按照监测方...目的总结并分析海南省近10年发生的常见过敏性疑似预防接种异常反应(adverse event following immunization,AEFI)案例图谱,摸索出切实有效的临床实用处理办法,为妥善处理过敏性异常反应积累案例经验。方法各级预防接种单位按照监测方案开展监测,通过监测系统网络直报,县级以上调查诊断专家组进行诊断和指导处置,收集相关图谱资料。结果建立了我省常见过敏性AEFI案例图例、调查诊断和处置方法资源库,对指导各级过敏性AEFI诊断及处理发挥关键作用。2008-2018年海南省报告发现常见过敏性反应2340例,居前五位的类型为:荨麻疹1866例(79.74%)、血管性水肿262例(11.20%)、斑丘疹108例(4.62%)、过敏性休克35例(1.50%)、过敏性紫癜和血小板减少性紫癜各13例(0.56%)。各种过敏性反应治疗效果都很好,治愈率100%,只有喉头水肿这一严重过敏反应死亡1例,治愈率80%。结论过敏性预防接种异常反应病例在AEFI中占比较大,是AEFI诊断处理很重要的工作内容,培训提高各级人员技能需临床治疗理论结合实际案例图谱分析才能达到好的效果。展开更多
BACKGROUND Moyamoya disease is essentially an ischemic cerebrovascular disease.Here,we describe a case of acute recurrent cerebral infarction caused by moyamoya disease with concurrent adenomyosis which,to our knowled...BACKGROUND Moyamoya disease is essentially an ischemic cerebrovascular disease.Here,we describe a case of acute recurrent cerebral infarction caused by moyamoya disease with concurrent adenomyosis which,to our knowledge,is the first in the literature.A literature review is also presented.CASE SUMMARY A 38-year-old female presented to the Research and Treatment Center of Moyamoya Disease in our hospital with"left limb weakness"as the main symptom.She was diagnosed with acute cerebral infarction and moyamoya disease through magnetic resonance imaging and digital subtraction angiography.Prior to this,she had experienced a prolonged menstrual period of one-month duration.This was investigated and adenomyosis was diagnosed.After passing the acute cerebral infarction phase,the patient underwent surgery for adenomyosis followed by combined cerebral revascularization.During the postoperative follow-up,improvements of the perfusion imaging stage and modified Rankin Scale were observed.A review of the literature showed only 16 reported cases of gynecological diseases complicated with stroke.The clinical characteristics,pathogenesis,therapeutic effects,and long-term prognosis of these cases have been studied and discussed.CONCLUSION In patients with moyamoya disease,early management of gynecological-related bleeding disorders is essential to prevent the complications of cerebral events.展开更多
BACKGROUND A Bochdalek hernia(BH)is a congenital diaphragmatic hernia that often develops in the neonatal period.BH typically occurs on the left side of the diaphragm.A right-sided BH in an adult is rare.CASE SUMMARY ...BACKGROUND A Bochdalek hernia(BH)is a congenital diaphragmatic hernia that often develops in the neonatal period.BH typically occurs on the left side of the diaphragm.A right-sided BH in an adult is rare.CASE SUMMARY A 45-year-old man was referred to our hospital because of an abnormal shadow seen on chest radiography during a medical check-up.A chest radiograph showed elevation of the right hemidiaphragm.Computed tomography showed prolapse of multiple intraabdominal organs into the right thoracic cavity,corresponding to a right-sided BH.The herniated contents included the stomach,transverse colon,and left lobe of the liver.The left lobe of the liver was enlarged,particularly the medial segment.Laparoscopic surgery was performed.However,the left lobe of the liver was completely trapped in the thoracic cavity.Therefore,thoracoscopic manipulation had to be performed to return the liver to the abdominal cavity.The hernia was repaired with interrupted nonabsorbable sutures and reinforced with mesh.CONCLUSION Combined laparoscopic and thoracoscopic surgery was successfully performed for right-sided BH with massive liver prolapse and abnormal liver morphology.展开更多

BACKGROUND Balanced translocation refers to the process where breakage and reconnection of chromosomes occur at abnormal positions.As the genetic substance with balanced translocation in individuals does not change,wh...BACKGROUND Balanced translocation refers to the process where breakage and reconnection of chromosomes occur at abnormal positions.As the genetic substance with balanced translocation in individuals does not change,which is usually characterized by normal phenotype and intelligence,the individuals seek medical service after many miscarriages,resulting in considerable mental and physical burdens of the family members.In the current era with rapid advances in detection technology,cytogenetic examination,as a definitive approach,still plays an essential role.CASE SUMMARY We report six cases with balanced chromosome translocation:Case 1:46,XY,t(3;12)(q27;q24.1),infertility after 3 years of marriage;Case 2:46,XX,t(4;16)(q31;q12),small uterus and irregular menstruation;Case 3:46,XY,t(4;5)(q33;q13),9qh+,not pregnant after arrested fetal development;Case 4:46,XX,t(11;17)(q13;p11.2),not pregnant after two times of spontaneous abortion;Case 5:46,XX,t(10;13)(q24;q21.2),not pregnant after arrested fetal development for once;Case 6:46,XX,t(1;4)(p36.1;q31.1),not pregnant after arrested fetal development for two times.The first four cases had chromosomal aberration karyotypes.CONCLUSION These results suggested that balanced chromosomal translocation carriers are associated with reproductive risks and a very high probability of abnormal pregnancy.The discovery of the first four reported chromosomal aberration karyotypes provides an important basis for studying the occurrence of genetic diseases.展开更多

We herein report a case of a 47-year-old woman with a 3-year history of constant diplopia as a consequence of multiple neurosurgical interventions due to hemorrhage from a cavernous angioma located in the subthalamic region. She also had a history of major depressive disorder and ulcerative colitis. The patient underwent a surgical intervention with a 5 mm bilateral recession of the superior and inferior rectus muscles. Five years after surgery, the patient reported less recurrent and prominent episodes of transient horizontal deviation with horizontal diplopia, with a prevalence of well-being and comfort.展开更多
