The thickness of coats can be used not only as a good age indicator for the geomorphic surfaces but also coats themselves can be directly used as dating materials in the ESR method. Through measuring the thickness of carbonate coats on stones in soils on the alluvial terraces in the mouth of the Heishanxia gorge of the Yellow River and ESR dating of layers separated from the carbonate coats, the average of accumulation rates of the thickness of carbonate coats on stones since 1.57 Ma was calculated to be 0.10 mm/ka in the studied area, and a regression equation between carbonate coat thickness and age was also generated. AIM:To describe the multimodal imaging features,treatment,and outcomes of patients diagnosed with adultonset Coats disease.METHODS:This retrospective study included patients first diagnosed with Coats disease at≥18 y...AIM:To describe the multimodal imaging features,treatment,and outcomes of patients diagnosed with adultonset Coats disease.METHODS:This retrospective study included patients first diagnosed with Coats disease at≥18 years of age between September 2017 and September 2021.Some patients received anti-vascular endothelial growth factor(VEGF)therapy(conbercept,0.5 mg)as the initial treatment,which was combined with laser photocoagulation as needed.All the patients underwent best corrected visual acuity(BCVA)and intraocular pressure examinations,fundus color photography,spontaneous fluorescence tests,fundus fluorescein angiography,optical coherence tomography(OCT),OCT angiography,and other examinations.BCVA alterations and multimodal image findings in the affected eyes following treatment were compared and the prognostic factors were analyzed.RESULTS:The study included 15 patients who were aged 24-72(57.33±12.61)y at presentation.Systemic hypertension was the most common associated systemic condition,occurring in 13(86.7%)patients.Baseline BCVA ranged from 2.0 to 5.0(4.0±1.1),which showed improvement following treatment(4.2±1.0).Multimodal imaging revealed retinal telangiectasis in 13 patients(86.7%),patchy hemorrhage in 5 patients(33.3%),and stage 2B disease(Shield’s staging criteria)in 11 patients(73.3%).OCT revealed that the baseline central macular thickness(CMT)ranged from 129 to 964μm(473.0±230.1μm),with 13 patients(86.7%)exhibiting a baseline CMT exceeding 250μm.Furthermore,8 patients(53.3%)presented with an epiretinal membrane at baseline or during follow-up.Hyper-reflective scars were observed on OCT in five patients(33.3%)with poor visual prognosis.Vision deteriorated in one patient who did not receive treatment.Final vision was stable in three patients who received laser treatment,whereas improvement was observed in one of two patients who received anti-VEGF therapy alone.In addition,8 of 9 patients(88.9%)who received laser treatment and conbercept exhibited stable or improved BCVA.CONCLUSION:Multi展开更多
AIM: To elucidate the association of treatment modality to vitreoretinal fibrosis and traction retinal detachment(TRD) in Coats' disease.METHODS: A PubMed search for Coats' disease with included studies descri...AIM: To elucidate the association of treatment modality to vitreoretinal fibrosis and traction retinal detachment(TRD) in Coats' disease.METHODS: A PubMed search for Coats' disease with included studies describing eyes with clinical features and treatment course of Coats' disease. Binary logistic regression with fibrosis at presentation and treatment type as independent variables was performed to determine predictors of TRD historically(since 1921) and in the antivascular endothelial growth factor(VEGF) era(since2007). Odds ratios(OR) with 95% confidence intervals(CI)reported.RESULTS: Of 175 articles described 1183 eyes. Vitreoretinal fibrosis increased from presentation(5.4%) to follow-up(15.5%) and TRD increased from 0.44% to 3.9% at follow up. Laser was protective against vitreoretinal fibrosis(OR0.6, 95%CI 0.4-0.9) but TRD was borderline(OR 0.6, 95%CI0.3-1.1). Cryotherapy showed a higher association with TRD(OR 1.9, 95%CI 1.0-3.7) than with vitreoretinal fibrosis(OR 0.8, 95%CI 0.5-1.2). Similarly, intravitreal anti-VEGF alone was not associated with fibrosis(OR 1.1, 95%CI 0.6-1.8) nor TRD(OR 1.1, 95%CI 0.5-2.6) but the combination of laser and anti-VEGF therapy was protective [Fibrosis: 0.1(0.03, 0.35); TRD: 0.05(0.01, 0.23)] compared to anti-VEGF plus cryotherapy(P<0.001). Disease stage ≤2 B or ≥3 A was not associated with TRD.CONCLUSION: Vitreoretinal fibrosis and TRD increase after treatment in Coats' disease. The combination of antiVEGF agents and cryotherapy may lead to higher risk for TRD. Best-corrected visual acuity was measured using theearly treatment of diabetic retinopathy study chart.RESULTS The study included eight eyes of 8 patients (7 men) aged 36.10 ± 6.65 years. Theaverage BCVA of the affected eye before treatment was 51.17 ± 15.15 letters(range, 28–70 letters), and the average central macular thickness was 303.30 ±107.87 μm (range, 221–673 μm). Four eyes were injected once, three were injectedtwice, and one was injected three times. Average follow-up duration was 37.33 ±2.26 mo. Average BCVA of the affected eye was 51.17 ± 15.15 letters before treatment and was increased by 13.50 ± 3.20, 16.25 ± 7.73, 18.25 ± 8.96, 18.03 ± 5.27,18.63 ± 3.35, 19.75 ± 6.96, 18.05 ± 5.36 and 17.88 ± 3.45 letters at 1 wk, 1 mo, 3 mo, 6mo, 9 mo, 12 mo, 24 mo and 36 mo after treatment, respectively (P < 0.01). Thepatients showed varying degrees of subretinal fluid resorption after treatment.None of the patients had serious complications such as increased intraocularpressure, development/progression of cataracts, endophthalmitis or retinaldetachment.CONCLUSION Intravitreal injection of conbercept combined with 532-nm laser photocoagulationmay be a feasible treatment for Coats disease in adul展开更多
The North China Plain(NCP)is troubled by severe haze pollution and the evolution of haze pollution is closely related to the atmospheric boundary layer(ABL).However,experimental and theoretical studies on the physical...The North China Plain(NCP)is troubled by severe haze pollution and the evolution of haze pollution is closely related to the atmospheric boundary layer(ABL).However,experimental and theoretical studies on the physical-chemical processes of the ABL in the NCP are lacking,with many scientific problems to be addressed.To solve these problems,the Comprehensive Observation on the Atmospheric boundary layer Three-dimensional Structure(COATS)during haze pollution was carried out in the NCP from 2016 to 2020.The COATS experiment adopted a"point-line-surface"spatial layout,obtaining both spatial-temporal profiles of the meteorological and environmental elements in the ABL and the turbulent transport data of fine particulate matter(PM_(2.5))in winter and summer.The research achievements are as follows.The spatial-temporal distribution characteristics of the ABL structure and PM_(2.5)concentrations in NCP were determined.The typical thermal structure of persistent heavy haze events and the pollutant removal mechanism by low-level jets were revealed.It was determined that the spatial structure of the ABL adjusted by the Taihang Mountains is responsible for the heterogeneous distribution of haze pollution in the NCP,and that mountain-induced vertical circulations can promote the formation of elevated pollution layers.The restraints of the atmospheric internal boundaries on horizontal diffusion of pollutants were emphasized.The contribution of the ABL to haze pollution in winter and summer was qualitatively compared and quantitatively estimated.The turbulent transport nature behind the relationship between the atmospheric boundary layer height(ABLH)and surface PM_(2.5)concentrations was analyzed.The concept of"aerosol accumulation layer"was defined,and the applicability of the material method in determining ABLH was clarified.A measurement system for obtaining the turbulent flux of PM_(2.5)concentrations was developed,and the turbulence characteristics of PM_(2.5)concentrations were demonstrated.The COATS experiment is of grea展开更多

AIM: To assess surgical outcomes of a novel method of transscleral drainage of subretinal fluid using a 25-gauge trocar-cannula with a self-closing valve(DTV) in patients with severe exudative retinal detachment(ERD) ...AIM: To assess surgical outcomes of a novel method of transscleral drainage of subretinal fluid using a 25-gauge trocar-cannula with a self-closing valve(DTV) in patients with severe exudative retinal detachment(ERD) in Coats disease.METHODS: Retrospective consecutive cases of 20 patients(20 eyes) of severe ERD due to Coats disease(stage 3 B) in total 156 Coats patients between June 2015 and April 2019 were included in this study. The participants were aged 1 to 10 y with a mean age of 3.50±1.79 y. The mean follow-up time were 11.9 mo. Subretinal fluid was drained transsclerally using a novel method of DTV. The height of the retinal detachment and the regression of abnormal vessels including telangiectasias and aneurysms were observed. Complications including vitreoretinal fibrosis, tractional retinal detachments(TRD), endophthalmitis, retinal holes, and hemorrhages were evaluated.RESULTS: Following surgeries, the patients showed the replacement of ERD and regression of telangiectatic retinal vessels observed with binocular indirect ophthalmoscopy. Six patients received retinal cryotherapies and 12 patients received laser photocoagulations following first external subretinal fluid drainage using DTV. All patients underwent intravitreal anti-vascular endothelial growth factor therapies to induce residual subretinal fluid absorption. During followups, 8 patients underwent a second drainage operation, 17 patients received retinal laser photocoagulations and 7 patients received cryotherapies. Vitreoretinal fibrosis was found in 7 patients and 6 patients underwent microinvasive vitrectomies during the follow-up period. Severe TRD, iatrogenic retinal holes, and hemorrhages were not found.CONCLUSION: The authors present a new therapeutic approach that successfully drains subretinal fluid in advanced stage 3 B Coats disease with severe ERD. This is a simple, safe and less invasive approach when compared with traditional managements. However, it should be strictly selected for patients with high bullous ERD close to the 展开更多
