目的探讨血清肺泡表面活性物质蛋白D(surfactant protein D,SP-D)、白细胞介素-17(inteleukin-17,IL-17)、白细胞介素-33(inteleukin-33,IL-33)水平与重症肺炎患儿肺损伤程度及预后的关系。方法选取笔者医院2018年4月~2021年2月收治的14...目的探讨血清肺泡表面活性物质蛋白D(surfactant protein D,SP-D)、白细胞介素-17(inteleukin-17,IL-17)、白细胞介素-33(inteleukin-33,IL-33)水平与重症肺炎患儿肺损伤程度及预后的关系。方法选取笔者医院2018年4月~2021年2月收治的148例重症肺炎患儿作为病例组,70例健康儿童作为对照组进行临床研究。比较两组儿童血清SP-D、IL-17、IL-33水平,并分析3项指标与病例组患儿急性肺损伤评分、急性生理与慢性健康评分(APACHEⅡ)的相关性,以及在重症肺炎患儿治疗结局中的预测价值。结果病例组血清SP-D、IL-17、IL-33水平显著高于对照组(P<0.05)。病例组儿童治愈出院121例,死亡27例。治愈组患儿的血清SP-D、IL-17、IL-33水平,APACHEⅡ、LPS评分均低于死亡组患儿(P<0.05)。重症肺炎患儿血清SP-D、IL-17、IL-33水平与APACHEⅡ、LPS评分均呈正相关(P<0.05)。血清SP-D、IL-17、IL-33预测患儿的治疗结局的AUC值分别为0.712、0.803、0.877。结论血清SP-D、IL-17、IL-33水平能在一定程度上反映重症肺炎患儿肺损伤及病情的严重程度,对于患儿治疗结局有一定的预测价值。展开更多

As new inflammatory mediators are characterized, it is important to understand their roles in sepsis. Interleukin 33(IL-33) is a recently described member of the IL-1 family that is widely expressed in cells of barrier tissues. Upon tissue damage, IL-33 is released as an alarmin and activates various types of cells of both the innate and adaptive immune system through binding to the ST2/IL-1 receptor accessory protein complex. IL-33 has apparent pleiotropic functions in many disease models, with its actions strongly shaped by the local microenvironment. Recent studies have established a role for the IL-33-ST2 axis in the initiation and perpetuation of inflammation during endotoxemia, but its roles in sepsis appear to be organism and model dependent. In this review, we focus on the recent advances in understanding the role of the IL-33/ST2 axis in sepsis.展开更多
