Gilbert's syndrome is characterized by a benign indirect hyperbilirubinemia.It has often been underestimated and undiagnosed because of its mild symptoms;al-though it is not as rare as was once believed when its f...Gilbert's syndrome is characterized by a benign indirect hyperbilirubinemia.It has often been underestimated and undiagnosed because of its mild symptoms;al-though it is not as rare as was once believed when its frequency was estimated using data originating from biochemical tests.Based on molecular techniques,the occurrence of Gilbert's syndrome has changed,increas-ing to 10% in the Caucasian population.This molecular defect was described,by Bosma et al,in 1995,and af-fects the promoter region of the UGT 1A1 gene.In this case report,our aim is to present a new combination of two molecular defects in a Greek patient with Gilbert' s syndrome.A 13-year-old Greek girl was examined for Gilbert's syndrome using molecular techniques,and an uncommon genotype was revealed comprising the rare mutation G71R in trans with A(TA)7TAA motif.TheG71R mutation according to the literature,as well as our epidemiological data,is rare in Caucasians,while it is common in Asian populations.This is the first case study in the Greek population to report a new genotype for Gilbert's syndrome manifestation in the Caucasian population.展开更多

Intermittent fasting(IF)is an intervention that involves not only dietary modific-ations but also behavioral changes with the main core being a period of fasting alternating with a period of controlled feeding.The dur...Intermittent fasting(IF)is an intervention that involves not only dietary modific-ations but also behavioral changes with the main core being a period of fasting alternating with a period of controlled feeding.The duration of fasting differs from one regimen to another.Ramadan fasting(RF)is a religious fasting for Muslims,it lasts for only one month every one lunar year.In this model of fasting,observers abstain from food and water for a period that extends from dawn to sunset.The period of daily fasting is variable(12-18 hours)as Ramadan rotates in all seasons of the year.Consequently,longer duration of daily fasting is observed during the summer.In fact,RF is a peculiar type of IF.It is a dry IF as no water is allowed during the fasting hours,also there are no calorie restrictions during feeding hours,and the mealtime is exclusively nighttime.These three variables of the RF model are believed to have a variable impact on different liver diseases.RF was evaluated by different observational and interventional studies among patients with non-alcoholic fatty liver disease and it was associated with improve-ments in anthropometric measures,metabolic profile,and liver biochemistry regardless of the calorie restriction among lean and obese patients.The situation is rather different for patients with liver cirrhosis.RF was associated with adverse events among patients with liver cirrhosis irrespective of the underlying etiology of cirrhosis.Cirrhotic patients developed new ascites,ascites were increased,had higher serum bilirubin levels after Ramadan,and frequently developed hepatic encephalopathy and acute upper gastrointestinal bleeding.These complications were higher among patients with Child class B and C cirrhosis,and some fatalities occurred due to fasting.Liver transplant recipients as a special group of patients,are vulnerable to dehydration,fluctuation in blood immunosuppressive levels,likelihood of deterioration and hence observing RF without special precautions could represent a real danger for them.Patients展开更多

Magnetic films with low Gilbert damping are crucial for magnonic devices,which provide a promising platform forrealizing ultralow-energy devices.In this study,low Gilbert damping and coercive field were observed in Bi...Magnetic films with low Gilbert damping are crucial for magnonic devices,which provide a promising platform forrealizing ultralow-energy devices.In this study,low Gilbert damping and coercive field were observed in Bi/In-dopedyttrium iron garnet(BiIn:YIG)thin films.The BiIn:YIG(444)films were deposited onto different substrates using pulsedlaser deposition.Low coercivity(<1 Oe)with saturation magnetization of 125.09 emu/cc was achieved along the in-planedirection of BiIn:YIG film.The values of Gilbert damping and inhomogeneous broadening of ferromagnetic resonance inBiIn:YIG films were obtained to be as low as 4.05×10^(-4)and 5.62 Oe,respectively.In addition to low damping,the giantFaraday rotation angles(up to 2.9×10^(4)deg/cm)were also observed in the BiIn:YIG film.By modifying the magneticstructure and coupling effect between Bi^(3+)and Fe^(3+)of Bi:YIG,doped In^(3+)plays a key role on variation of the magneticproperties.The low damping and giant Faraday effect made the BiIn:YIG film an appealing candidate for magnonic andmagneto-optical devices.展开更多

This case highlights a patient with Gilbert syndrome who underwent endoscopic retrograde cholangiopancreatography(ERCP) with removal of bile duct stones, who then experienced an unexplained increase in bilirubin, with...This case highlights a patient with Gilbert syndrome who underwent endoscopic retrograde cholangiopancreatography(ERCP) with removal of bile duct stones, who then experienced an unexplained increase in bilirubin, with total bilirubin(TBIL) levels increasing from 159.5 μmol/L to 396.2 μmol/L and to a maximum of 502.8 μmol/L after 9 d. Following the decrease in the TBIL level, enhanced magnetic resonance cholangiopancreatography(MRCP) was performed to exclude any possible remaining choledocholithiasis. Nevertheless, the serum bilirubin level increased again, with TBIL levels rising from 455.7 μmol/L to 594.8 μmol/L and a maximum level of 660.3 μmol/L with no remaining bile duct stones. A liver biopsy showed severe bile duct cholestasis with no inflammation. Based on the exclusion of other potential causes of hyperbilirubinemia and the fact that both instances of increased bilirubin occurred after ERCP and MRCP, the contrast agents iopromide and gadoterate meglumine were suspected to be the causes of the hyperbilirubinemia. As of the writing of this report, the patient's bilirubin levels have spontaneously returned to baseline levels. In summary,ERCP and MRCP utilizing the contrast agents iopromide and gadoterate meglumine may possibly induce prolonged hyperbilirubinemia.展开更多
