目的比较腹腔镜胆总管切开取石术(laparoscopic common bile duct exploration,LCBDE)中一期缝合与T管引流在胆总管结石患者中的应用效果。方法回顾性分析2017年2月至2019年2月上海健康医学院附属周浦医院收治的82例胆总管结石患者,并...目的比较腹腔镜胆总管切开取石术(laparoscopic common bile duct exploration,LCBDE)中一期缝合与T管引流在胆总管结石患者中的应用效果。方法回顾性分析2017年2月至2019年2月上海健康医学院附属周浦医院收治的82例胆总管结石患者,并根据手术方案将其分为A组(39例)和B组(43例),A组采用LCBDE术中一期缝合治疗,B组采用LCBDE术中T管引流治疗;统计两组的手术时间、术中出血量、拔管时间、胃肠道恢复时间以及住院总时间;比较两组治疗前、后的血清白细胞介素-1β(IL-1β)、C反应蛋白(CRP)和肿瘤坏死因子-α(TNF-α)等炎性因子水平变化情况及两组术后总并发症发生率。结果 A组的手术时间、胃肠道恢复时间以及住院时间均短于B组,差异具有统计学意义(P<0.05),两组患者的术中出血量、拔管时间比较,差异无统计学意义(P>0.05);A组治疗后,血清IL-1β、CRP和TNF-α水平均低于B组,差异具有统计学意义(P<0.05);A组的总并发症发生率(7.69%)与B组(13.95%)比较,差异无统计学意义(P>0.05)。结论 LCBDE术中一期缝合相比T管引流能够显著缩短手术时间、胃肠道恢复时间以及住院时间,改善患者炎性因子表达水平。展开更多

In this study, SHIV-CN97001 was in vivo pa...SHIV-CN97001 played an important role in assessing the immune effect and strategy of the AIDS vaccine which included genes of the predominant prevalent HIV-1 strain in China. In this study, SHIV-CN97001 was in vivo passaged serially to construct pathogenic SHIV-CN97001/rhesus macaques model. To identify variation in the gp120 region of SHIV-CN97001 during passage, the fragments of gp120 gene were amplified by RT-PCR from the plasma of SHIV-CN97001 infected animals at the peak viral load time point and the gene distances (divergence, diversity) were calculated using DISTANCE. The analysis revealed that the genetic distances of SHIV-CN97001 in the third passage animals were the highest during in vivo passage. It had a relationship between viral divergence from the founder strain and viral replication ability. The nucleic acid sequence of the V3 region was highly conservative. All of the SHIV-CN97001 strains had V3 loop central motif (GPGQ) and were predicted to be using CCR5 co-receptor on the basis of the critical amino acids within V3 loop. These results show that there was no significant increase in the genetic distance during serial passage, and SHIV-CN97001 gp120 gene evolved toward ancestral states upon transmission to a new host. This could partly explain why there was no pathogenic viral strain obtained during in vivo passage.展开更多
The discovery of cuprate high TC superconductors has inspired the search for unconventional superconductors in magnetic materials.A successful recipe has been to suppress long−range order in a magnetic parent compound...The discovery of cuprate high TC superconductors has inspired the search for unconventional superconductors in magnetic materials.A successful recipe has been to suppress long−range order in a magnetic parent compound by doping or high pressure to drive the material towards a quantum critical point.We report an exception to this rule in the recently discovered potassium iron selenide.The superconducting composition is identified as the iron vacancy ordered K_(0.83)(2)Fe_(1.64)(1)Se_(2) with T_(C) above 30 K.A novel large moment 3.31μB/Fe antiferromagnetic order that conforms to the tetragonal crystal symmetry has an unprecedentedly high ordering temperature TN≈559 K for a bulk superconductor.Staggeringly polarized electronic density of states is thus suspected,which would stimulate further investigation into superconductivity in a strong spin-exchange field under new circumstances.展开更多

By analyzing a simplifi...This paper presents a control algorithm for push recovery, which particularly focuses on the hip strategy when an external disturbance is applied on the body of a standing under-actuated biped. By analyzing a simplified dynamic model of a bipedal robot in the stance phase, it is found that horizontal stability can be maintained with a suitably controlled torque applied at the hip. However, errors in the angle or angular velocity of body posture may appear, due to the dynamic coupling of the transla- tional and rotational motions. To solve this problem, different hip strategies are discussed for two cases when (1) external dis- turbance is applied on the center of mass (CoM) and (2) external torque is acting around the CoM, and a universal hip strategy is derived for most disturbances. Moreover, three torque primitives for the hip, depending on the type of disturbance, are designed to achieve translational and rotational balance recovery simultaneously. Compared with closed-loop control, the advantage of the open-loop methods of torque primitives lies in rapid response and reasonable performance. Finally, simulation studies of the push recovery of a bipedal robot are presented to demonstrate the effectiveness of the proposed methods.展开更多
BACKGROUND The treatment of benign biliary strictures(BBS)is a challenging clinical problem.At present,there is a lack of ideal models for the study of BBS treatment.AIM To develop a novel animal model of BBS to simul...BACKGROUND The treatment of benign biliary strictures(BBS)is a challenging clinical problem.At present,there is a lack of ideal models for the study of BBS treatment.AIM To develop a novel animal model of BBS to simulate studies on the processes and mechanisms in the human condition.METHODS A rabbit model of benign bile duct stricture was established by surgical injury of the bile duct.After removal of the gallbladder,a drainage tube was placed th-rough the cystic duct at the stump,and a BBS model was induced by surgical injury at the lower end of the common bile duct.RESULTS Compared with the control group,the model rabbits showed gross jaundice,increased serum bilirubin,and decreased liver function.Cholangiography showed segmental bile duct stenosis in the model rabbits.Pathological staining showed inflammatory cell infiltration and fibrosis in the biliary tract of rabbits in the model group.This was consistent with the clinical manifestations of BBS.This model provided serology,imaging,pathology,and other aspects of BBS.CONCLUSION We have successfully established an animal model of benign stricture of the lower bile duct with repeatable administration,which is consistent with the clinical manifestations of BBS.展开更多
Background:Complex noise and its relation to hearing loss are difficult to measure and evaluate.In complex noise measurement,individual exposure results may not accurately represent lifetime noise exposure.Thus,the me...Background:Complex noise and its relation to hearing loss are difficult to measure and evaluate.In complex noise measurement,individual exposure results may not accurately represent lifetime noise exposure.Thus,the mean LAeq,8 h values of individuals in the same workgroup were also used to represent LAeq,8h in our study.Our study aimed to explore whether the mean exposure levels of workers in the same workgroup represented real noise exposure better than individual exposure levels did.Methods:A cross-sectional study was conducted to establish a model for cumulative noise exposure (CNE) and hearing loss in 205 occupational noise-exposed workers who were recruited from two large automobile manufacturers in China.We used a personal noise dosimeter and a questionnaire to determine the workers' occupational noise exposure levels and exposure times,respectively.A qualified audiologist used standardized audiometric procedures to assess hearing acuity after at least 16 h of noise avoidance.Results:We observed that 88.3% of workers were exposed to more than 85 dB(A) of occupational noise (mean:89.3 ± 4.2 dB(A)).The personal CNE (CNEp) and workgroup CNE (CNEg) were 100.5 ± 4.7 dB(A) and 100.5 ± 2.9 dB(A),respectively.In the binary logistic regression analysis,we established a regression model with high-frequency hearing loss as the dependent variable and CNE as the independent variable.The Wald value was 5.014 with CNEp as the independent variable and 8.653 with CNEg as the independent variable.Furthermore,we found that the figure for CNEg was more similar to the stationary noise reference than CNEp was.The CNEg model was better than the CNEp model.In this circumstance,we can measure some subjects instead of the whole workgroup and save manpower.Conclusions:In a complex noise environment,the measurements of average noise exposure level of the workgroup can improve the accuracy and save manpower.展开更多

TBM can be better used in factories by non-professional workers...Background Task-based measurement (TBM) is a method to assess the eight-hour A-weighted equivalent noise exposure level (LAeq. 8h) besides dosimeter. TBM can be better used in factories by non-professional workers and staffs.However, it is still not clear if TBM is equal or similar with dosimeter for LAeq.8h measurement in general. This study considered the measurement with dosimeter as real personal noise exposure level (PNEL) and assessed the accuracy of TBM by comparing the consistencies of TBM and dosimeter in LAeq.8h measurement.Methods The study was conducted in one automobile firm among 387 workers who are exposed to unstable noise.Dosimeters and TBM were used to compare the two strategies and assess the degree of agreement and causes of disagreement. Worker's PNEL was measured via TBM for noise; the real PNEL was also recorded. The TBM for noise was computed with task/position noise levels measured via sound level meter and workers' exposure information collected via working diary forms (WDF) filled by participants themselves. Full-shift noise exposure measurement via personal noise dosimeters were taken as the real PNEL. General linear model (GLM) was built to analyze the accuracy of TBM for noise and the source of difference between TBM for noise and real PNEL.Results The LAeq.8h with TBM were slightly higher than the real PNELs, except the electricians. Differences of the two values had statistical significance in stamping workers (P <0.001), assembly workers (P=0.015) and welding workers (P=0.001). The correlation coefficient of LAeq.8h with TBM and real PNELs was 0.841. Differences of the two results were mainly affected by real PNEL (F=11.27, P=0.001); and work groups (F=3.11, P <0.001) divided by jobs and workshops were also independent factors.
