摘要
目的 观察复方利多卡因乳膏联合利多卡因及罗哌卡因针的麻醉效果,并比较环形皮下注射与传统阴茎根部背神经阻滞的麻醉效果.方法 170例行包皮环切术患者,随机分为A组(对照组,利多卡因针阴茎根部背神经阻滞麻醉)、B组(药物改良组,利多卡因乳膏外用,利多卡因针及罗哌卡因针阴茎根部背神经阻滞麻醉)、C组(注射方法改良组,利多卡因乳膏外用,利多卡因针及罗哌卡因针阴茎根部环形皮下注射).分别比较各组的麻醉起效时间和持续时间,并比较术后各时间段的VAS评分.结果 各组麻醉起效时间无明显差异,但A组的麻醉持续时间明显低于B组和C组.在麻醉时,C组VAS值明显低于A组及B组(P<0.05),A组和B组间差异无统计学意义(P>0.05).术中,三组评分均无明显差异.至术后30min、1h、2h,A组均高于B组及C组(P<0.05).结论 麻醉时,阴茎根部环形皮下注射,患者的疼痛感明显轻于背侧根部背神经阻滞麻醉.复方利多卡因乳膏,利多卡因加盐酸罗哌卡因针联合麻醉,持续时间长,术后的疼痛程度较单纯使用利多卡因轻.
Objective To observe the analgesia effect of compound lidocaine cream associating with lidocane and ropivacaine in circumcision resection. To compare the analgesia effect of ring-like subcutaneous injections and dorsal nerve block in the root of penis. Methods 170 circumcision patients were randomly divided into Group A(Lidocane,dorsal nerve block in the root of penis);Group B(Compound Lidocaine Cream smearing, dorsal nerve block in the root of penis with lidocane and ropivacaine);Group C(Compound Lidocaine Cream smearing,ring-like subcutaneous injections with lidocane and ropivacaine). Onset time of anesthesia,duration of anesthesia and VAS scores of each group were evaluated. Results There was no significant difference in the onset time of anesthesia in each group,but the duration of anesthesia in Group A was significant lower than that in Group B and C. In anesthetizing,VAS score of Group C was lower than Group A and B(P<0.05),but no significant differences between Group A and B(P>0.05). There were no differences among three groups during the operation. At 30 minutes,1 hour,2 hours after the operation,the score Group A was higher than Group B and C(P<0.05). Conclusion The pain of patients of ring-like subcutaneous injections is significantly lighter than that of the dorsal nerve block in the penis root. Postoperative pain of compound lidocaine cream smearing,injections with lidocaine and ropivacaine,is lighter than using lidocaine alone.
