摘要
目的：探讨低年资护士的护理临床决策能力水平及其影响因素，为低年资护士临床决策能力的培养提供理论依据。方法应用自行设计的一般资料问卷、护理临床决策能力量表，对633名低年资护士进行问卷调查。结果低年资护士护理临床决策能力总分为（143.55±14.01）分，各维度得分中寻找解决问题的方法得分最低，为（30.98±4.80）分，评价实施效果得分最高，为（37.31±4.53）分；单因素分析结果显示，医院级别、工作目的、学术活动、护患关系、是否胜任护理工作对护理临床决策能力的影响差异有统计学意义（F=3.089～21.694，P＜0.05或0.01）；多因素分析结果显示，工作年限、婚姻状况、学术活动、护患关系、是否胜任护理工作是护理临床决策能力的影响因素。结论低年资护士的护理临床决策能力水平有待提高，护理管理者及教育者应关注工作年限少、未婚护士特点，干预可控因素，弥补培训缺口，传授护患沟通技巧，增强低年资护士的护理临床决策能力，提高护理服务质量。
Objective To investigate the influencing factors for clinical decision-making ability of junior nurses, and provide theoretical basis for the cultivation of clinical decision-making ability of junior nurses. Methods A total of 633 junior nurses were investigated by using the general information questionnaire and the clinical decision-making ability questionnaire. Results The gross score in clinical decision-making ability of junior nurses was (143.55 ± 14.01) points, they scored lowest on the dimension of finding the solution to the problem, the score was (30.98±4.80) points, and they scored highest on the dimension of evaluating implementation effect, the score was (37.31±4.53) points. The unilabiate analysis revealed that the influence on the clinical decision-making ability of hospital level, job purpose, academic activity, relationship between nurses and patients, whether competent in nursing work were statistically significant (F=3.089-21.694, P<0.05 or 0.01). The multivariate analysis revealed that the factors impacting the nursing clinical decision-making ability were work experience, marital status, academic activity, relationship between nurses and patients, whether competent in nursing work. Conclusions The clinical decision-making ability of junior nurses remains to be improved, and the nursing administrators and educators should pay more attention to the different features of the unmarried nurse or the one lacking in work experience, intervene the controllable factors, make up the training gap, and teach the communication technique between nurses and patients to improve the clinical decision-making ability and the quality of nursing service.
