巨细胞病毒感染患儿听力损害危险因素分析 被引量:4

Analysis on risk factors of hearing impairment on children with cytomegalovirus infection

摘要 目的 探讨影响巨细胞病毒(CMV)感染儿童听力损害的危险因素.方法 回顾性分析2013-2015年在温州医科大学附属第二医院育英儿童医院住院的275例CMV感染患儿的临床资料和脑干诱发电位(ABR)检测结果,采用x2检验和多因素Logistic回归分析法探讨影响CMV感染患儿听力损害的危险因素.结果 275例(550耳)患儿中听力异常73例(双侧异常34例),共107耳,其中轻度异常79耳(占73.8%),中度异常16耳,重度异常8耳,极重度异常4耳.多因素Logistic回归分析显示:男性(OR =2.042,95%CI:1.097~3.802)、头颅MRI异常(OR=2.999,95%CI:1.251~7.186)是CMV感染患儿听力损害的独立危险因素;肝炎(OR=0.113,95%CI:0.020~0.637)、血小板减少性瘀斑(OR=0.217,95%CI:0.056~0.842)与听力损害呈负相关.结论 CMV感染的男性及头颅MRI异常的患儿更容易出现听力损害,而出现肝炎或血小板减少性瘀斑时或有助于早期发现CMV感染,从而获得有效治疗减少对患儿听力的损害,但确切机制有待进一步研究. Objective To explore the risk factors of hearing impairment after cytomegalovirus (CMV) infected in children.Methods A total of 275 hospitalized cases of children with CMV infected were collected from the Second Affiliated Hospital & Yuying Children's Hospital of Wenzhou Medical University during 2013 and 2015.The clinical data and auditory brainstem response (ABR) results were retrospectively analyzed.Chi-square test and Logistic regression analysis were performed to identify the risk factors of hearing impairment after CMV infected.Results In 275 cases (totally 550 ears),there were 73 cases with hearing impairment (34 cases lost hearing in both ears) and totally 107 ears,among which 79 ears were mild abnormality,16 ears were medium abnormality,8 ears were serious abnormality and 4 ears were extremely serious abnormality.Multifactors Logistic regression analysis showed that male (OR=2.042,95%CI:1.097-3.802) and cranial MRI abnormal (OR=2.999,95%CI:1.251-7.186) were independent risk factors for hearing impairment on children with CMV infected.The negative correlation were observed between hepatitis (OR=0.113,95% CI:0.020-0.637),thrombocytopenia (OR =0.217,95%CI:0.056-0.842) and hearing impairment.Conclusions CMV infected male children and cranial MRI abnormal children combine with hearing impairment more common.The emergence of hepatitis and thrombocytopenic ecchymosis may contribute to the early detection and treatment of the disease so as to reduce the hearing impairment,but the exact mechanism needs further research.
