围术期使用艾司氯胺酮在加速康复外科的应用进展

摘要 随着加速康复外科理念的不断推广和普及,围术期的操作均有目的的遵循如何加速康复。而疼痛是临床常见影响康复的并发症之一,疼痛不仅能刺激交感神经,引发一系列应激反应,还能影响神经内分泌功能,导致术后精神心理状态改变和内环境紊乱,是影响患者康复的重要因素。多模式镇痛和减少术后并发症越来越被重视。围术期使用艾司氯胺酮是多模式镇痛的重要策略之一,可以明显减少阿片类药物用量,有效提高术后镇痛满意率,预防并减轻术后并发症,加速患者康复。本文基于加速康复外科理念,对艾司氯胺酮药理学特点、临床疗效以及在围术期的临床应用进展进行综述。 With the developing popularization of the concept of enhanced recovery after surgery (ERAS), perioperative procedures pay attention to accelerating rehabilitation. It is pain that is one of the common complications which affects rehabilitation in clinical practice. Not only can the sympathetic nervous system be stimulated and a series of stress reactions be led, but also the neuroendocrine function can be influenced, which will bring some changes in mental and psychological status and internal environment disorder after surgery, therefore pain is an important factor affecting the re-habilitation of patients. Multi-mode analgesia and reducing postoperative complications are in-creasingly being valued. Using esketamine during the perioperative period is one of the important strategies for multimodal analgesia, which can significantly reduce the usage of opioids, effectively improve the satisfaction rate of postoperative analgesia, prevent and reduce postoperative compli-cations, and accelerate the recovery of patients. Based on the concept of accelerated rehabilitation surgery, this article reviews the pharmacological characteristics, clinical efficacy, and clinical appli-cation progress of ketamine in perioperative period.
