摘要
为了探讨抗核抗体 (ANA)阳性肝硬化患者的临床特点及环磷酰胺的治疗效果 ,检测 82例肝硬化患者血清ANA ,比较ANA阴性和阳性肝硬化患者的临床特点 ,观察环磷酰胺治疗ANA阳性肝硬化患者的疗效。结果与ANA阴性肝硬化患者比较 ,ANA阳性肝硬化患者女性多见 ,发病年龄较大 ,乙型肝炎病毒感染率低 ,Child -Pugh分级C级比例高 ;腹水、下肢浮肿症状较重 ;血清白蛋白水平低、球蛋白水平高 (P <0 0 5 )。ANA阳性肝硬化患者经环磷酰胺治疗后症状均好转 ,其中与对照组比较 ,腹水、下肢浮肿症状改善明显 (P <0 0 5 ) ;血清总蛋白、白蛋白水平增加 ,球蛋白水平下降 (P <0 0 5 ) ;部分患者ANA转阴 (P <0 0 5 )。说明ANA阳性肝硬化患者有其自身的临床特点 ,环磷酰胺治疗有效。
To investigate clinical features in cirrhosis patients with serum positive antinuclear antibody(ANA) and the treating effect of cyclophosphamide on these patients. The serum ANA level was tested in patients with cirrhosis. Clinical features of cirrhosis patients with serum positive ANA were compared with negative, and the treating effect of cyclophosphamide on cirrhosis patients with serum positive ANA was investigated. Compared with the cases with negative ANA, the cases with serum positive ANA and more female, lower infected rate of hepatitis B virus and higher proportion of Child-Pugh hierarchical C grade (P<0.05), in which the symptoms of ascites and legs edema were worse significantly (P<0.05)and whose age were older (P<0.05);The serum albumin and total globulin levels in cirrhosis patients with positive ANA dropped significantly. After having been treated with cyclophosphamide in cirrhosis patients with serum positive ANA, ascites and legs edema symptoms were improved significantly (P<0.05),serum albumin level rose and globulin level dropped significantly (P<0.05). The cirrhosis patients with positive ANA have theirs clinical distinguishing features and cyclophosphamide is an effective therapeutic medication.
