摘要
应用WBD—Ⅰ、Ⅱ型微波刀行肝切除70例,微波刀有良好的组织凝固及止血性能,术中失血量较传统切肝方法明显减少,平均每例失血量249ml,输血量295ml,30例术中不需输血。全组无手术并发症及死亡。详细介绍了手术操作方法及注意事项,对微波刀的工作原理及操作过程中的一些问题进行讨论。
The microwave tissue coagulator has been applied clinically with satisfactor results. 2450-MHz microwave f or medical use are generated and transmitted to a needle type electrode and it is calld microwave scalpel by us. this electrode is thrust directly into the liver tissue and this is repeated along the line were incision is anticipated. between Janura 1990 and April 1992 microwave scalpel was used in 70 patients having hepatic resection for avariety ot condition consist of: Primary hepatocellular carcinoma 46 cases; Hemangioma 18 cases; Hepatobiliary tract stone 2 cases; Huge cystadenoma of the biliary 1 case; Inflammatory pseudotumor of the liver 1 case; Metastasis carcionma of the Liver 2case. Hemostasis was excellent despite the presence of cirrihosis in all patients. The average amounts of blood loss and blood transfusion were 249ml and 205ml respectively. 30 patients did not need blood transfusion. all cases were free from postoperstive bleeding from resected liver stump and infection of abdominal cavity. No complication attributable to microwave coagulation were noted. These finding led us to conclude that this new operative technique can be utilized safely and easily in the field of hepatic surgery.
