摘要
目的 研究房颤患者体表心电图中QTc、STc、Tc的变化 ,并试图由此变化推测细胞离子流的改变。方法 采用 12导联同步心电图测量 15 3例房颤患者和 88例窦性心律对照组的QT、RR、ST、T值 ,以Bazett公式分别对QT、ST、T进行校正 ,并进行组间差异性比较。同时对冠心病及高血压两亚组的房颤与窦性心律者相应参数也进行比较。结果 房颤组的QTc为 4 4 0 .6 1± 4 .2 7ms,STc为 16 1.19± 3.2 6ms,Tc为 192 .2 7± 2 .88ms;对照组的QTc、STc、Tc分别为 4 2 5 .13± 3.35 ,15 7.89± 3.0 9,172 .89± 3.77ms .房颤组QTc延长 ,Tc延长 ,与对照组相比具有显著差异 ;STc在房颤组也延长 ,但无统计学意义。亚组间显示相同结果。结论 房颤患者体表心电图中代表心肌复极的参数QTc、Tc延长 ,机制可能与心肌细胞钾离子通道重构有关。
Objective To observe the repolarization parameters QT interval and ST-T change in patients with atrial fibrillation and to study the possible ionic basis of surface ECG abnomalities.Methods QT interval,ST segment duration and T-wave duration were measured through 12-leads electrocardiograms in 153 AF patients (80were CHD,29 were hypertension,the other were rheumatics) and 88 age-matched sinus subjects as control group.All these variables were corrected for heart rate by Bazett's formula.Results QTc,STc,Tc of AF were 440.61±4.27 ms,161.19±3.26ms,and 192.27±2.88 ms respectively,and QTc,STc,Tc of controls were 425.13±3.35,157.89±3.09,and172.89±3.77 ms.QTc,Tc of AF were significantly prolonged,STc had no significant difference between AF and control group.Conclusion Repolarization parameters QTc and Tc are prolonged in AF patients,and it is proposed that the changing of potassium channel (Iks,Ikr) might be involved in the initiation and perpetuation of AF.
