摘要
目的 :探讨瘤腔内化疗对骨巨细胞瘤远期疗效的影响。方法 :回顾分析随访时间 5年以上的 13例患者 ,男 6例 ,女 7例。中位年龄为 2 7岁 (2 0～ 61岁 )。胫骨上端 6例 ,股骨下端 3例 ,股骨上端 2例 ,跟骨和尺骨近端各1例。Jaffe分级 :Ⅰ级 2例 ,Ⅱ级 8例 ,Ⅲ级 3例 ;Campanacci分期 :Ⅰ期 4例 ,Ⅱ期 7例 ,Ⅲ期 2例。均采用肿瘤刮除、瘤腔灭活、植骨、瘤腔内化疗的方法治疗。结果 :所有患者均获得 5～ 11年的随访。 2例复发 ,其中 1例因肺转移而死亡。肢体功能评分 3 0分 10例 ,2 6分 1例 ,2 4分 1例。结论 :瘤腔内化疗是一种治疗骨巨细胞瘤可行的方法 ,主要适应于瘤腔完整的骨巨细胞瘤。
Objective:To study the effects of intralesional chemotherapy on the long-term outcome of giant cell tumor of bone (GCT).Method:Thirteen patients (6 males,7 females) were followed-up over 5 years,with the median age of 27 years old.Six were in the proximal tibia,3 in the distal femur,2 in the proximal femur,one in the proximal ulna and one in calcaneus.Two patients were diagnozed as GCT Ⅰ grade histologically,8 as GCT Ⅱ grade,3 as GCT Ⅲ grade.According to the grading standard of GCT X-ray findings,4 were in Ⅰ grade,7 in Ⅱ grade and 2 in Ⅲ grade.All were treated with tumor curettage,adjuvant phenol therapy,bone grafting and intralesional chemotherapy with MTX.Result:All patients were followed-up for 5～11 years.Two recurred and one of them died because of lung metastasis.The limb functional evaluation after surgical treatment of GCT were done on 12 patients,and 10 of them were in 30 points,each one in 26 points and 24 points.Conclusion:The curettage,phenol therapy,bone grafting and intralesional chemotherapy with MTX is an feasible treatment for GCT with intact tumor cavity.
