一种新型记忆合金椎间融合器的拔出强度研究 被引量:1

摘要 目的 :测试并评价记忆合金椎间融合器的拔出强度。方法 :12具新鲜尸体腰椎标本 ,分为记忆合金椎间融合器组 (SMAcage)、螺纹状椎间融合器组 (Interfix)、方形融合器组 (Prospace)。每组测试 4个L3、4节段的双侧共 8枚融合器的拔出强度。结果 :SAMcage、Interfix和Prospace的最大拔出强度为 (75 0± 2 0 5 )N、 (695± 175 )N和 (2 5 8± 65 )N。SMAcage的拔出强度与Interfix相比无统计学差异 (P =0 2 4) ;但明显高于Prospace (P =0 0 17)。根据不同拔出曲线求得的拔出斜率分别为 (2 19 7± 49)N/mm (SMAcage)、 (195 6± 75 )N/mm (Interfix)、 (5 0 4± 2 4)N /mm (Prospace)。SMAcage和Interfix之间差别不显著 (P =0 5 9) ;与Prospace则有统计学差异 (P =0 0 2 5 )。结论 :SMAcage在用于测试的 3种融合器中具有最佳的抗拔出性能。 Objective:To test the pullout strength of a newly designed shape memory alloy lumbar interbody fusion cage and compare with that of threaded cage and rectangular cage.Method:Twelve cadaver lumbar segments (L 3、4 ) were harvested to evaluate pullout strength.They were divided into three groups:(1)SMAcage,(2)Cylindrical threaded cage (Interfix,SOFAMOR-DANEK),(3)rectangular cage with ridges on its surface (Prospace,AESCULAP).Each specimen had 2 cages inserted from both sides.Different cages were tested by a biomechanical test machine(MTS858 Mini Bionix).Result:The SMAcages required a median pullout force of (750±205)N,as compared with the lower pullout force of the Interfix (695±175)N,and the Prospace (258±65)N.There were no statistical difference between SMAcage and Interfix ( P=0.24 ).Significant difference was observed between SMAcage and Prospace ( P=0.017 ).The pullout curves of three cages had their own unique characteristic.The slope of each curves were (219.7±49)N/mm (SMAcage)、(195.6±75)N/mm (Interfix)、(50.4±24)N/mm(Prospace),there were no significant difference between SMAcage and Interfix( P=0.59 ),but a statistical difference between SMAcage and Prospace ( P=0.025 ).Conclusion:The newly designed shape memory alloy cage shows highest pullout resistance among the three types of cages because of its anchorage device at the upper and lower surface.
