摘要
【目的】评价三维超声在肝脏疾病介入性诊疗操作中的应用价值。【方法】肝病患者 5 7例 ,介入性诊疗操作 72次 ,用二维及三维超声观察穿刺针并判断穿刺针与病灶的位置关系。【结果】72次操作中 ,三维超声对针尖的显示优于二维超声 (P <0 .0 5 ) ,对针杆的显示与二维超声无明显差异 (P >0 .0 5 )。三维超声在判断穿刺针与病灶的位置关系时 ,信心水平明显提高 (P <0 .0 1)。 72次操作中 ,三维超声观察到 12次(16 .7% )操作中针尖位置不理想 ,需要重新穿刺或调整针尖位置。【结论】三维超声在肝脏疾病介入性诊疗操作中能更好地发挥导向作用。
To evaluate the application of two- and three-dimensional ultrasonography(2D-US and 3D-US) in interventional procedures of diagnosis and treatment for liver diseases.2D-US and 3D-US for 72 interventional procedures: visualization of the puncture needles and judgment of the positional relationship between needle and liver lesions in 57 patients with liver diseases were undertaken.The clarity of needle tip visualization was improved in 3D-US images as compared with that of 2D-US images(P<0.05), whereas there was no difference between two modalities in visualization of needle shaft(P>0.05). The confidence level of the operator in judging positional relationship between needle and lesions was enhanced when 3D-US was performed(P<0.01). In twelve (16.7%) out of 72 procedures, 3D-US detected improper needle placement, re-puncture or readjustment for the position of needle tips was required.[Conclusion]3D-US can play an important role in guiding interventional procedure for diagnosis and treatment of liver diseases.
