全血环孢素浓度对T淋巴亚群和移植肾功能的影响 被引量:3

摘要 本文讨论了50例肾移植术后病人全血环孢素浓度对T淋巴亚群和移植肾功能状态的影响。用TDx法测定全血环孢素浓度并同时用间接荧光免疫法测外周血T淋巴亚群:CD_3^+,CD_4^+,CD_8^+。将T淋巴亚群资料按不同环孢素峰、谷浓度分组,用t检验进行统计学处理。结果表明:在环孢素峰浓度过高(>1500μg/l)与过低组(<600μg/l)之间,CD_8^+,CD_4^+/CD_8^+值有非常显著差异(p<0.001);环孢素谷浓度过高(>800μg/l),过低(<400μg/l)时所发生的移植肾功能恶化次数远多于环孢素谷浓度处于中间范围者。本文认为:环孢素血浓度对T淋巴亚群以及移植肾的功能有明显的影响;环孢素峰、谷浓度在其作用中起不同贡献。因此建议临床不仅测定环孢素谷值,还要测定其峰值,并将它们控制在适当的范围里,以保护移植肾,延长其存活时间,井减轻环孢素对机体的过度免疫抑制作用及毒性。 The influence of cyclosporine concentrations on the T lymphocyte subset and renal function in 50patients receiving cyclosporine after renal transplantation was studied. The date of T lymphocyte subset were classifiedinto different groups according to respective peak and trough cyclosporine concentrations and dealt with t test. The resultsshowed that there were very significant difference in CD_8^+ and CD_4^+/CD_8^+ between peak concentration low (0-600μg/l) and high (above 1500μg/l) group; the incidence of renal function worsening was the fewest in middle group of cy-closporine concentrations (peak:600-1500μg/l; trough: 400-800μg/l). It is concluded that different peak and troughcyclosporine concentrations had different influence on T lymphocyte subset and renal function, and it is recommendedthat the peak and trough concentrations should be limited to certain range to minimize the side effect and maximize thetherapeutic effect.
