EXPRESSION OF P16, cyclinD1 AND PCNA PROTEIN IN PANCREATIC ENDOCRINE TUMORS

摘要 ①目的 探讨胰腺内分泌肿瘤 (PET)组织P1 6、cyclinD1和增殖细胞核抗原 (PCNA)蛋白的表达及其意义。②方法 应用免疫组化S P法 ,检测P1 6、cyclinD1和PCNA蛋白在 32例PET和 1 5例正常胰腺组织中的表达。③结果 P1 6蛋白在PET中的阳性表达率为 2 8.1 % ,在正常胰腺中为 86 .7% ,二者比较 ,差异有显著性 (χ2 =1 4 .0 6 ,P <0 .0 1 )。cyclinD1蛋白在PET中过表达率为 5 6 .3% ,而在正常胰腺未见过表达 ,二者比较差异有显著性(χ2 =1 3.6 7,P <0 .0 1 ) ;cyclinD1过表达组PCNA标记指数 (PCNALI)明显高于cyclinD1低表达组 (t =5 .76 ,P <0 .0 1 )。恶性PET的PCNALI显著高于良性PET(t=5 .32 ,P <0 .0 1 )。P1 6和cyclinD1蛋白在PET中的表达呈负相关 (r =- 0 .6 2 ,P <0 .0 1 )。④结论 P1 6蛋白表达缺失与cyclinD1蛋白过度表达可能在PET的发生中发挥协同作用 ,PCNA的高表达与cyclinD1过度表达可能与PET的恶性转化有关。 Objective To evaluate the expression and significance of P16, cyclinD1 and PCNA protein in pancreatic endocrine tumors(PET). Methods S-P immunohistochemical staining was performed to detect the expression of P16, cyclinD1 and PCNA protein in 32 PETs and 15 of normal pancreas. Results The positive expression rate of P16 protein in PET was 28.1%, which was significantly lower than that in normal pancreatic islets (86.7%)(χ 2=14.06, P<0.01). Overexpression of cyclinD1 protein was detected in 56.3% of PETs, but not found in normal pancreatic islets (χ 2=13.67, P<0.01). PCNA labeling index (PCNA LI) of the PETs which had overexpression of cyclinD1 protein was significantly higher than that of the PETs which had negative or low expression of cyclinD1 protein (t=5.76, P<0.01). PCNA LI of advanced PETs was significantly higher than that of benign ones (t=5.32, P<0.01). Of 23 PETs which had positive expression of cyclinD1 protein, there were 20 PETs which had negative expression of P16 protein. Whereas six PETs had positive expression of P16 protein in nine PETs with negative expression of cyclinD1 protein. There was a negative correlation between the expression of cyclinD1 and P16 (r=-0.62, P<0.01). Conclusion The loss of P16 and overexpression of cyclinD1 may work in coordination in the genesis of PET. The high expression of PCNA and overexpression of cyclinD1 may be related to the malignant transformation of PET.
