摘要
作者采用氩激光对230例视网膜格子样变性者作预防性治疗和长时间的随访观察,同时对109例未作激光治疗的同样患者进行比较性研究。结果发现:做过激光预防性治疗者(1)视网膜脱离的患病率(1.3％)明显低于未治疗者(12.8％),P<0.01;(2)视网膜新裂孔的患病率(3.4％)明显低于未治疗者(17.4％),P<0.01;(3)视网膜格子样变性恶化发展的患病率(5.2％)明显低于未治疗者(22.0％),P<0.01。文章对激光作预防性治疗的适应证和光凝方式进行了分析讨论。
This paper presents the follow-up results and comparative study on the prophylactictreatment with argon laser photocoagulation. The 230 fellow eyes with retinal degenerationfollowing one eye suffering from retinal detachment were photocoagulated using spot size200μm, power 0.2～0.4W and the duration 0.1～0.5s. The other 109 eyes untreated wereused as a control group. The results were as follows: (1) the retinal detachment inciden-ce in the treated group was much lower than that of untreated group (1.3% and 12.8%, res-pectively, P<0.01); (2) the number of new retinal break could be decreased followingphotocoagulation (3.4% and 17.4% respectively,P<0.01); (3) the tendency of spread of thelesions were reduced (5.2% and 22.0% respectively,P<0.01); (4) there was no significantdifference in the effect in entire closure and partial closure (9.02% and 8.2% respectively,P>0.05). The photocoagulation effeciency was inspiring. We suggest, however, that not allpatients should be given this prophylactic treatment. More attention should be paid to thefollowing conditions: (1) myopia or higher myopia; (2) formation of vitreoretinalbreaks; (3) larger vitreoretinal adhering areas; (4) retinal lattice degeneration associatedwith posterior vitreous detachment.
