摘要
目的 :介绍应用“田氏脊柱骨刀”行单侧小切口关节突间开窗治疗腰椎间盘突出症的手术方法。方法 :皮肤切口长 3cm ,单侧暴露关节突 ,“U”形截骨切除上一椎骨的下关节突显露下一椎骨的上关节突关节面 ,再行“L”形截骨切除下一椎骨的上关节突 ,切除黄韧带暴露硬膜管和神经根 ,剥开神经根切除椎间盘。术毕放置橡皮条引流 ,分层闭合切口。结果 :1992～ 2 0 0 2年采用此手术方法治疗腰椎间盘突出症 10 0例 ,收到满意效果 ,达到切口小、出血少、手术时间短 ,术后恢复快等微创手术的要求。结论 :这种手术方法为微创式的手术方法。值得推广应用。
Objective: To introduce the operative method of unilateral micro-incisive articular processus fenestration for treatment of lumbar disc herniation by Tian's spinal osteotomes. Methods: To make a length of 3cm incision on the skin, and from unilateral to expose the articular processus, then used of the Tian's osteotomes to make a U type osteotomy, which was resected the inferior articular processus of upper vertebra, the facet of superior articular processus of lower vertebra was exposed, to make the L type osteotomy once again, which was resected the superior articular processus of lower vertebra and resection of yellow ligament to exposed the dural sac and nerve root, then pushed aside the nerve root to resected the lumbar disc. Postoperation, setting the drainage tube and closed the incision. Results: From 1992～2002 years,there were 100 cases of lumbar disc herniation treated by this way, all of the patients achieved a satisfactory effect,the main advantages of this way was small incision, less hemorrhage, shorten operative time, quick recovery of postoperation. This way was in conformity with character of micro-traumatic operation. Conclusion:This way is a micro-traumatic operative method. It's worth of popularization.
