摘要
目的 :探讨上消化道肿瘤病人术后早期肠内营养的可行性及安全性 ,以促进病人早日康复。方法 :将收治的 5 2例食管癌及胃贲门癌病人随机分为早期肠内营养组 (EEN)3 0例及延迟肠内营养组(DEN) 2 2例 ,EEN组采用液体型全营养制剂能全力 ,经鼻肠管或空肠营养管 ,通过输液泵输入 ,DEN组则行全胃肠外营养 3d～ 5d后 ,再逐渐过渡到肠内营养。观察两种营养方式对术后恢复的影响。结果 :两组肛门排气时间、平均住院日及术后并发症对比 ,有统计学意义 (P <0 .0 5 )。结论 :上消化道肿瘤病人术后早期采用肠内营养 ,肠功能恢复较快 。
Objective: in order to accelerate earlier recovery of patients with upper digestive tract tumor, to probe into both the feasibility and safety for them to undergo early stage postoperative enteral nutrition. Method: a total of 52 cases of esophagus carcinoma or cardiac cancer patients were selected and divided into early-stage enteral nutrition (EEN) group and delayed enteral nutrition (DEN) group randomly. Patients of EEN group (30 cases) were given early stage post-operative enteral nutrition that was liquids total nutrition preparation “Nengquanli' given via nasal-intestinal feeding tube or via infusion pump. Patients of DEN group (22 cases) were accepted total parenteral nutrition for 3 days to 5 days, then gradually changed to enteral nutrition. Influence of the two kinds of nutritional supports on post-operative recovery of the two group patients had been observed. Results: anal exsufflation times of post-operative patients were 67.2 h ±13.92 h in EEN group and 79.3 h ± 15.66 h in DEN group. Average hospitalization times of post-operative patients were 12.8 days in EEN group and 16.6 days in DEN group. Moreover, patients of EEN group had less post-operative complications than that of DEN group. There was significant statistical difference between the two groups (P<0.05). Conclusion: to give early stage enteral nutritional support for patients with upper digestive tract tumor may accelerate the recovery of intestinal function and decrease post-operative complications of them.
