摘要
目的 :观察断流和分流联合术治疗门静脉高压症的效果。方法 :总结采用断流加分流治疗 13例门静脉高压症的经验。结果 :择期手术 10例 ,预防性手术 3例 ,均无手术死亡。出院病例均随防至今 ,无手术后再出血 ,发生肝性脑病 2例 ,均存活。术前门静脉压力平均为 (3.6 8± 0 .16 )kPa ,术后为 (3.18± 0 .11)kPa。结论 :联合手术具有断流和合理口径分流的特点 ,可将断流和分流两者优缺点互补 ,是目前治疗门静脉高压症的较为理想术式。
Objective:To evaluate the effect of combined pericardiac devasculariztion and shunt for treating portal hypertension.Methods:The data of 13 cases of portal hypertension treated with portoazygous devascularization and shunt from April 1996 to June 2001 was summarized.Results:There was no death in all 13 cases,10 cases receiving selective operation,3 cases receiving preventive operation.The portal pressure before and after operation was ( 3.68±0.16)kPa and ( 3.18±0.11)kPa,respectively.All the patients were followed up,no rebleeding occured,but 2 cases had hepatic encephalopathy,and no death happened.Conclusion:Combined pericardiac devascularization and shunt is characterized by complete devascularization of sustaining varices and distal partial shunt with reasonable diameter.A combination of both shunt and devascularization,it is one of the ideal choices currently available for treating portal hypertension.
