小儿惊厥血清和脑脊液中NSE和NO/NOS改变及其临床意义 被引量:6

摘要 目的 探讨小儿惊厥发作 ,发作时间 ,引起惊厥原因对脑神经元的损伤及其程度。方法 按照引起小儿惊厥的病因及惊厥发生持续的时间及次数进行分组 ,分别为五组、三组和三组。应用酶联免疫法和分光光度法测定惊厥发作后不同时间患儿血清和脑脊液中NSE ,NO/NOS的含量。结果 ①除热性惊厥组外 ,所有惊厥患儿血清及脑脊液中NSE ,NO/NOS明显高于正常对照组 (P <0 0 5 ,P <0 0 1) ;②无论任何原因引起惊厥 ,惊厥时间越长 ,血清和脑脊液中NSE ,NO/NOS越高 ,两者成正相关 (r =80 5~ 82 4,P <0 0 1) ;③在所有病因中 ,颅内感染者由于惊厥平均次数最多及平均持续时间最长 ,故脑脊液及血清中NSE ,NO/NOS水平升高最明显 ,且持续时间最长 ,尤其是有昏迷或严重神经系统并发症者尤为突出 ,而热性惊厥者NSE ,NO/NOS则无明显升高或只有轻微升高。结论 惊厥发作时由于伴有不同程度神经元损伤 ,从而使血清和脑脊液中NSE ,NO/NOS水平其升高 ,其程度与惊厥发作持续时间 。 Objective To explore the relationship of neuron injury and its severity with the cause and the duration of convulsion Methods Patients were divided into 5 groups or 3 groups according to the causes of convulsion or duration of convulsion in children The levels of neuron specific enolase(NSE) and nitrogen monoxide and nitricoxide synthase(NO/NOS) in serum and cerebrospinal fluid(CSF) were tested by ELISA and spectrophotography at different time points after convulsion in children Results The levels of NSE and NO/NOS in serum and CSF of all objective groups except the febrile convulsion group were apparently higher than those in control group( t =15 4~47 7, P <0 05~0 01) No matter what the causes might be, the levels of NSE and NO/NOS in serum and CSF increased in a time dependent manner There was a positive correlation between them( r =80 5~82 4, P <0 01) Because of more average times and longer duration of convulsion, the levels of NSE and NO/NOS in serum and CSF in children with intracranial infection were the highest and lasted for the longest time in all convulsion induced causes, especially in those with coma and severe nervous system complications However, no increase or just slight increase of the levels of NSE and NO/NOS was found in serum and CSF in children with febrile convulsion Conclusion Convulsion accompanied by different degrees of neuron injury leads to increased levels of NSE and NO/NOS in serum and CSF which are positively correlated with the duration and causes of convulsion and the severity of the brain damage
