摘要
目的:探讨消痔灵注射液对混合痔选择性痔上黏膜吻合术(TST)患者肛肠动力及并发症的影响。方法 :选取我院2017年4月—2018年4月收治的178例混合痔TST手术患者,按随机数字表法分为观察组和对照组,各89例。对照组患者给予常规消炎治疗,观察组患者给予消痔灵注射液(6~13 mL,qd)治疗。比较两组治疗前后肛肠动力学指标[直肠耐受量、直肠感觉阈、肛管静息压、直肠静息压]变化情况、并发症[伤口疼痛、肛门坠胀、创缘水肿、伤口感染、伤口出血]及随访1年后混合痔复发的情况。结果 :治疗后,观察组肛管静息压高于对照组,直肠耐受量、直肠感觉阈及直肠静息压均低于对照组,差异有统计学意义(P <0.05);观察组并发症发生率小于对照组,差异有统计学意义(P <0.05);随访1年,观察组混合痔复发率为2.25%,低于对照组的10.11%,差异有统计学意义(P <0.05)。结论 :采用消痔灵注射液治疗混合痔TST手术患者的疗效显著,可有效改善患者的肛肠动力学指标,减少并发症的发生,降低复发率。
Objective:To investigate the effect of xiaozhiling injection on anorectal motility and complications in patients mixed hemorrhoids undergoing tissue-selecting therapy stapler(TST)operation.Methods:178 patients mixed hemorrhoids undergoing TST operation from April 2017 to April 2018 in our hospital were selected and randomly divided into an observation group and a control group,89 cases each.The patients in the control group were given conventional anti-inflammatory treatment,and the patients in the observation group were treated with xiaozhiling injection(6-13 m L,qd).The changes in anorectal dynamics indexes[rectal tolerance,rectal sensation threshold,anal canal resting pressure,rectal resting pressure],complications[wound pain,anal dropping flatulence,wound edge edema,wound infection,wound hemorrhage]and the recurrence of mixed hemorrhoids after 1 year of follow-up were compared between the two groups.Results:After treatment,the anal canal resting pressure in the observation group was higher than that in the control group,while the rectal tolerance,rectal sensory threshold and rectal resting pressure were lower than those in the control group,with statistically significant differences(P<0.05).The incidence of complications in the observation group was lower than that in the control group,with statistically significant difference(P<0.05).The recurrence rate of mixed hemorrhoids after 1-year follow-up was 2.25%in the observation group,which was lower than 10.11%in the control group(P<0.05).Conclusion:Xiaozhiling injection is significantly effective in the treatment of patients with mixed hemorrhoids undergoing TST operation,which can effectively improve the anorectal dynamics indexes of the patients,reduce the incidence of complications and reduce the recurrence rate.
