喜辽妥用于改善血管条件及提高PICC穿刺成功率的效果观察 被引量:5

摘要 目的:分析喜辽妥在外周静脉穿刺中心静脉置管术(PICC)穿刺中对血管条件及穿刺成功率的影响。方法:选择2015年10月—2016年4月于我院进行PICC穿刺的100例患儿为研究对象,按随机数字表法分为喜辽妥组与山莨菪碱组,各50例。喜辽妥组:确定血管部位并以此为中心均匀涂抹喜辽妥,范围:直径≥5 cm,用量:1 cm/次×部位;频率:5次/d;使用时间:3~10 d。山莨菪碱组:山莨菪碱(1 m L∶10 mg)全部浸入直径≥5 cm的无菌棉纱中并将该棉纱敷于所选血管部位的中心位置,10 min后取下无菌棉纱。用量:10 m L(100 mg);频率:5次/d;使用时间:3~10 d。操作者于实施干预后记录两组血管等级、穿刺耗时、一次穿刺是否成功情况。结果:干预1 d,2 d时山莨菪碱组血管改善例数均多于喜辽妥组,差异具有统计学意义(P<0.05);干预7~10 d时喜辽妥组血管改善例数均高于山莨菪碱组,差异有统计学意义(P<0.05)。山莨菪碱组、喜辽妥组的一次穿刺成功率分别是64.00%,84.00%,二次穿刺成功率分别是72.00%,98.00%,喜辽妥组高于山莨菪碱组,差异有统计学意义(P<0.05)。山莨菪碱组PICC置管耗时为(8.94±3.55)min,喜辽妥组为(5.21±1.78)min,喜辽妥组低于山莨菪碱组,差异有统计学意义(P<0.05)。结论:喜辽妥可改善血管条件、提高PICC穿刺成功率、降低穿刺耗时。 Objective: To analyze the effect of hirudoid on vascular conditions and the success rate of puncture in peripherally inserted central venous catheterization(PICC) puncture. Methods:100 cases of children undergoing PICC puncture in our hospital from October 2015 to April 2016 were selected as subjects and randomly divided into mucopolysaccharide polysulfate cream group and anisodamine group, 50 cases in each group. In the hirudoid group, vascular site was determined and hirudoid was evenly daubed with vascular site as center, range: diameter ≥ 5 cm, dosage: 1 cm/times × site; frequency: 5 times/day; using time: 3~10 days. In the anisodamine group, anisodamine(1 m L∶10 mg) was immersed completely in sterile cotton yarn with diameter≥5 cm, and the cotton yarn was applied to the center of the selected vascular and then removed 10 minutes later, dosage: 10 m L(100 mg); frequency: 5 times/day; using time: 3~10 days. After the intervention, the grade of the blood vessel, puncture time and whether the one-time puncture was successful were recorded by the operator. Results:The number of cases of vascular improvement in the anisodamine group was significantly more than that in the hirudoid group on the first day and the second day of intervention(P<0.05); the number of cases of vascular improvement in the hirudoid group was significantly more than that in the anisodamine group from the 7 th day to 10 th day of intervention(P<0.05). The success rate of one-time puncture in the anisodamine group and hirudoid group was 64.00% and 84.00%, respectively, and the success rate of secondary puncture was 72.00% and 98.00%, respectively, so the success rate was significantly higher in the hirudoid group than the anisodamine group(P<0.05). The PICC catheterization time in the anisodamine group was(8.94±3.55) min, while it was(5.21±1.78) min in the hirudoid group, so the PICC catheterization time in the hirudoid group was significantly lower than that in the anisodamine group(P<0.05).
