摘要
目的观察自拟化痰泻火方治疗女童特发性性早熟(ICPP)的疗效和对性激素水平、卵巢容积及骨龄/实际年龄(BA/CA)的影响。方法选取丽水市人民医院儿科2017年1月至2018年8月收治的ICPP女童150例,按照随机数字表法分为治疗组和对照组,各75例。治疗组给予自拟化痰泻火方治疗,对照组给予曲普瑞林治疗,2组均以6个月为1个疗程,治疗1个疗程后观察临床疗效、性激素水平、乳房Tanner评分、卵巢容积和BA/CA值的变化情况。结果治疗组总有效率为94.66%,显著高于对照组的80.00%,2组比较,差异有统计学意义(P<0.05)。治疗前,2组血清促黄体生成素(LH)、促卵泡成熟激素(FSH)及雌激素(E2)水平比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组LH、FSH、E2水平均较同组治疗前明显降低(P<0.05),且治疗组LH、FSH降幅优于对照组,差异有统计学意义(P<0.05)。治疗前,2组乳房Tanner评分、卵巢容积、BA/CA值比较,差异均无统计学意义(P>0.05),具有可比性;治疗后,2组上述指标均较同组治疗前显著降低,差异有统计学意义(P<0.05)。结论自拟化痰泻火方治疗ICPP女童疗效确切,可有效降低患儿性激素水平,缓解乳房硬结和胀痛,抑制乳房、卵巢增大及骨龄增长,值得临床推广应用。
Objective To observe the curative effects of self-made Huatan Xiehuo Fang(化痰泻火方)on girls with idiopathic central precocious puberty(ICPP)and its effects on sexual hormone levels,ovarian volume and bone age/actual age(BA/CA).Methods A total of 150 girls with ICPP admitted to department of pediatrics of Lishui the People’s Hospital from January 2017 to August 2018 were divided into treatment group and control group according to the random number table method,with 75 cases in each.The treatment group was given self-made Huatan Xiehuo Fang,while the control group was given Western medicine triptorelin.The 2 groups were both treated for 6 months as 1 course of treatment.After 1 course of treatment,the clinical efficacy,sexual hormone levels,breast Tanner score,ovarian volume and the changes of BA/CA value were compared between the two groups.Results The total effective rate of the treatment group was 94.66%,which was significantly higher than 80.00%of the control group,and the difference was statistically significant between the two groups(P<0.05).Before treatment,there was no statistically significant difference in the levels of sex hormones luteinizing hormone(LH),follicular maturation hormone(FSH)and estrogen(E2)between the two groups(P>0.05),and they were comparable.After treatment,the levels of LH,FSH and E2 in the two groups were significantly lower than those of the same group before treatment.The levels of LH and FSH in the treatment group were significantly lower than those in the control group(P<0.05),moreover,the decrease extent of LH and FSH in the treatment group was superior to that of the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the breast Tanner score,ovarian volume and BA/CA value between the two groups before treatment(P>0.05),and they were comparable.After treatment,the above-mentioned indexes of the two groups all significantly decreased compared with the same groups before treatment,and the difference was statisticall
