摘要
目的探讨颅后窝拥挤与原发性三叉神经痛(trigeminal neuralgia,TN)之间的相关性。方法回顾性研究60例原发性TN病例资料,同时将60例年龄、性别匹配的健康志愿者作为对照组。受试者均行高分辨率三维MRI检查(包括3DFIESTA、3D-TOF MRA序列)并使用3D-Slicer软件对原数据进行三维重建并测量颅后窝容积(posterior fossa volume,PFV)、后脑体积(hindbrain volume,HBV),颅后窝拥挤指数(posterior fossa crowdednessindex,PFCI)为HBV/PFV×100%。采用SPSS22.0统计软件进行比较。结果 TN病人平均PFCI为(85.0±3.9)%,而对照组为(82.7±3.9)%,两者具有显著统计学差异(P=0.025)。TN女性病人颅后窝比男性更拥挤,PFCI分别为(86.4±3.8)%、(83.4±3.4)%(P=0.033)。多元线性回归分析显示较高PFCI与女性(P=0.022)、较小年龄(P=-0.003)和TN病人(P=-0.023)有关。结论 TN病人的颅后窝比健康对照组更拥挤,PFCI较高与女性、年龄较小和患有TN有关。颅后窝拥挤可能是神经血管冲突的危险因素,因此,更容易导致TN发生。
Objective To explore the correlation between posterior fossa crowdedness and primary trigeminal neuralgia(TN).Methods The clinical data of 60 patients with primary TN were analyzed retrospectively and 60 age-and sex-matched healthy volunteers served as control group.All subjects underwent high-resolution three-dimensional MRI(including 3 D-FIESTA,3 D-TOF MRA sequence),and the3 D-Slicer software was used to reconstruct and measure the posterior fossa volume(PFV)and hindbrain volume(HBV).The posterior fossa crowdedness index(PFCI)was calculated as HBV/PFV×100%.Finally,all of the data collected were analyzed by SPSS 22.0 software.Results The mean PFCI was 85.0±3.9%in patients with TN and 82.7±3.9%in control group.There was a significant difference between the two groups(P=0.025).The women had a more crowded posterior fossa than men,and the PFCI was 86.4±3.8%and 83.4±3.4%respectively(P=0.033)in patients with TN.The multiple linear regression analysis showed that a higher PFCI was associated with woman(P=0.022),younger age(P=-0.003)and TN disease(P=-0.023).Conclusions Patients with TN have a more crowded posterior fossa than healthy controls.A higher PFCI is associated with women,younger age and TN disease.The posterior fossa crowdedness may be a risk factor of neurovascular conflict,and thus being more likely to result in the increase of the incidence of TN.
