摘要 目的 检测胃粘膜组织K ras、P16基因及表皮生长因子 (EGF) ,探讨其临床意义。方法 K ras、P16基因采用单链构象多态性分析法 (SSCP) ,EGF采用免疫组化法。结果 K ras在早期胃癌和进展期胃癌的阳性表达率分别为 5 0 %、75 % ,突变率分别为 78.6 %、72 .7% ;P16基因在早期胃癌和进展期胃癌的阳性表达率分别为 2 1.4 %、18.2 % ,突变率分别为 6 4 .3%、6 3.6 %。与对照组 (慢性浅表性胃炎 )相比差异均有显著性。EGF在有淋巴结转移和无淋巴结转移的胃癌组织中阳性表达分别为 76 .9%、38.9% ,两者有显著差异 (P <0 .0 5 )。结论 K ras、P16。 Objective To detect the expression of K-ras, P16 gene and EGF in gastric mucus tissue, and explore its clinical significance.Methods K-ras and Pl6 gene were detected by SSCP, and EGF was detected by immunohistochemical method.Results The percentage of K-ras and P16 positive expression cases in the patients with early gastric cancer was 50% and 21.4% respectively, while the percentage of gene mutation cases was 78.6% and 64.3% respectively.The percentage of K-ras and P16 positive expression cases in the patients with progressive stage of gastric cancerwere 75% and 18.2%, while the percentage of gene mutation cases was 72.7% and 63.6% respectively. In contrast, the results in the patients with chronic superficial gastritis (control group) were significantly lower than the above finding.The percentage of EGF-positive cases in the tissue of gastric carcinoma patients with lymph node metastasis and without metastasis was 76.9%and 38.9% respectively. A significant difference was found between the two group(P<0.05=.Conclusions The combined detection for expression and mutation of genes associated with cancers, such as K-ras and P16 as well as EGF is valuable for the early diagnosis and prognosis of gastric carcinoma.
