摘要
目的 观察危重病患者外周血单个核细胞核因子 -κB(NF -κB)的活性和序贯器官衰竭估计 (SOFA)评分的变化 ,探讨NF -κB对危重病患者病情和预后的预测作用。方法 将入住ICU的 4 0例危重病患者分成多器官功能障碍综合征(MODS)组和非MODS组 ,存活组和死亡组 ;选择健康体检者 2 0例作为对照组。检测他们外周血单个核细胞NF -κB的活性 ,并进行SOFA评分。结果 危重病患者的NF -κB活性明显高于对照组 (P <0 0 1) ;NF -κB的活性和SOFA评分在MODS组和死亡组分别明显高于非MODS组和存活组 (P <0 0 1) ;NF -κB的活性和SOFA评分呈明显正相关 (r =0 90 2 ,P <0 0 1)。结论 外周血单个核细胞NF -κB的活性是判断危重病患者病情和预后的敏感指标。
Objective To observe the changes of nuclear factor Kappa B(NF-κB) in peripheral blood mononuclear cells (PBMC)and sequential organ failure assessment (SOFA)scores in severely ill patients,and to study the role of NF-κB in estimating the state of illness and predicting outcome.Methods Forty patients in the intensive care unit (ICU) were in the trial group,and 20 healthy people were in the control group.Forty patients were divided into multiple organ dysfunction syndrome (MODS)group and non-MODS group according to the MODS diagnostic criterion,and they also were followed-up for 28 days and then assigned to the group of survivors and non-survivors,respectively.The activations of NF-κB in PBMC were measured by immunohistochemical method,and SOFA scores were calculated.Result The activation of NF-κB in severely ill patients was higher than that in control group(P<0.01).The activation of NF-κB and SOFA scores in the MODS group and the non-survivors group were higher than those in non-MODS group and the survivors group (P<0.01).There were evidently positive correlations between the activations of NF-κB and the SOFA scores(r=0.902,P<0.01).Conclusions Measurement of the activation of NF-κB in PBMC can be used to evaluate the progress and prognosis of severely ill patients during ICU stay.
