摘要
目的 :在新西兰大白兔建立腓肠神经小隐静脉岛状筋膜皮瓣模型 ,探讨浅静脉干在远、近端蒂皮瓣中的作用。方法 :将 2 0只兔随机分为 4组 ,每组 10个皮瓣 ,面积 6.0cm× 2 .0cm ,蒂宽 1.5cm ,长宽比例 4∶1。组 1为保留小隐静脉 (流出 )的近端蒂皮瓣 ,组 2为结扎小隐静脉的近端蒂皮瓣。组 3为保留小隐静脉 (流入 )的远端蒂皮瓣 ,组 4为结扎小隐静脉的远端蒂皮瓣。结果 :组 1皮瓣成活率 ( 88.8% )与组 2( 62 .6% ,P <0 .0 0 1)和组 3 ( 2 2 .7% ,P <0 .0 0 1)有极显著差别 ,组 4( 5 5 .5 % )与组 3 (P <0 .0 1)有显著差别 ,组 2与组 4无显著差别 (P >0 .0 5 )。组织切片组 3小隐静脉粗大僵硬 ,管腔充满血栓。结论 :浅静脉干血流导出对皮瓣成活有正向作用 (近端蒂 ) ,浅静脉干血流导入对皮瓣成活有负向作用 (远端蒂 )。
Objective: To establish a rabbit model of veno-neuro-fasciocutaneous flap and investigate the effects of large superficial veins in proximally-and distally-based flaps. Methods: The lesser saphenous sural veno-neuro-adipofascial pedicled fasciocutanous flap (6.0 cm×2.0 cm in size with pedicle width of 1.5 cm, length/width ratio 4∶1) was successfullly established in New Zealand White rabbit.Twenty rabbits were randomly allocated into 4 groups of 10 flaps. Group I was proximally veno-neuro-adipofascial based flaps with the lesser saphenous vein intact (outflow vein) in the pedicle.Group Ⅱ was also proximally-based flaps but the lesser saphenous vein was ligated at 1 cm proximal to the pedicle.Group Ⅲ was distally veno-neuro-adipofascial based flaps with the lesser saphenous vein intact (inflow vein) in the pedicle. Group Ⅳ was also distally-based flaps but the lesser saphenous vein was ligated at 1 cm distal to the pedicle. Flap survival rate was measured in 10 days.The lesser saphenous vein in the pedicle was harvested and examed histologically.Results: The mean flap survival rate of group Ⅰ (88.8%) was statistically higher than that of group Ⅱ(62.6%,P<0.001) and group Ⅲ (22.7%, P<0.001).The mean survival of group Ⅳ (55.5%) was significantly better than that of group Ⅲ (P<0.01).But group Ⅱ and group Ⅳ has no significant difference (P>0.05). Histological examination showed the lesser saphenous vein in group Ⅲ was extremely dilated and filled fully with thrombosis.Conculsion: Large superficial veins have positive effects (venous outflow) in proximally-based flaps, but negative effects (venous inflow) in distally-based flaps.
