摘要
目的观察肝血管瘤剥除术治疗肝血管瘤的疗效及对患者远期恢复、生存质量的影响.方法回顾性分析58例肝血管瘤患者的临床资料,根据其手术方式分为肝血管瘤剥除术组(A组)和规则性肝切除术组(B组).观察两组患者的手术情况、术后近远期恢复情况和生存质量改善情况.结果 A组患者各指标、肝功能恢复时间及近远期并发症发生率显著小于B组(P<0.05),两组术后复发率比较差异无统计学意义(P>0.05).术后12个月时,两组GLQI各维度评分均较术前均有显著提升,且A组大于B组(P<0.05).结论肝血管瘤剥除术治疗肝血管瘤手术情况、术后近远期恢复情况及生存质量改善效果均优于规则性肝切除术,对于瘤体位于肝脏周边、远离肝内大血管的肝血管瘤可优先选择肝血管瘤剥除术.
Objective To observe the efficacy of hepatic hemangioma enucleation in treating of hepatic hemangiomas and its effect on long-term recovery and quality of life. Method The clinical data of 58 patients with hepatic hemangiomas were retrospectively analyzed,and the patients were divided into hepatic hemangioma enucleation group( group A) and regular hepatectomy group( group B) according to the different surgical methods.The operation conditions,the short-term and long-term recovery and the quality of life improvement after operation were observed in the two groups. Results The operative time,hepatic portal occlusion time,intraoperative blood loss,liver function recovery time and the incidence rate of short-term and long-term complications in group A were significantly lower than those in group B( P < 0. 05). There was no significant difference in the recurrence rate after operation between two groups( P>0.05).At 12 months after operation,the GLQI scores in all dimensions in the two groups were significantly higher than those before operation,and the scores in group A were greater than those in group B( P <0.05). Conclusion Hepatic hemangioma enucleation has superior effects than regular hepatectomy in the aspects of hepatic hemangioma surgery,and short-term and long-term recovery and quality of life improvement after operation.For hepatic hemangiomas in which tumors are located in the periphery of liver and away from the great vessels in liver,hepatic hemangioma enucleation can be preferentially selected.
