结直肠癌中HMGA2表达的意义与微卫星不稳定性的关系 被引量:6

摘要 目的探讨高迁移率族蛋白(HMG) A2在结直肠癌组织中表达意义及与微卫星不稳定性(MSI)的关系。方法结直肠癌手术标本70例,采用免疫组化法检测微卫星不稳定四种相关蛋白MLH1、MSH2、MSH6、PMS2及HMGA2的表达情况,分为高频微卫星不稳定组(MSI-H)、低频微卫星不稳定(MSI-L)/微卫星稳定组(MSS)两组,研究HMGA2与MSI的相关性及与结直肠癌临床特征之间的关系,进一步用Western印迹法验证免疫组化结果。结果结直肠癌中HMGA2阳性率为65. 71%(46/70),与正常肠黏膜相比差异显著(P<0. 001),低分化癌组高于中、高分化癌组,差异有意义(P<0. 05); TNMⅢ+Ⅳ期明显高于Ⅰ+Ⅱ期(P<0. 05)。MSI-H发生率为17. 14%(12/70),右半结肠发生率高于左半结肠(P<0. 05),此外,MSI-H发生与患者年龄、肿瘤分化程度有关(P<0. 05)。MSI-H组、MSI-L/MSS组中HMGA2的表达差异无统计学意义(P>0. 05)。结论HMGA2高表达可能促进结直肠癌的发生与发展,作用机制与微卫星状态无关,需要进一步研究。 Objective To study the expression of the high mobility group protein( HMG) A2 in colorectal cancer and its rela-tionship with microsatellite instability( MSI).Methods 70 cases of colorectal cancer from the third hospital of Jinzhou medical univer-sity were divided into high-frequency microsatellite instability( MSI-H),low-frequency microsatellite instability( MSI-L)/microsatellitestable( MSS) groups. The expressions of MLH1,MSH2,MSH6,PMS2 and HMGA2 were detected by immunohistochemistry,the cor-relation between HMGA2 and MSI and the relationship with the clinical features of colorectal cancer were analyzed,the results of immu-nohistochemistry were further confirmed by Western blot.Results The positive rate of HMGA2 in colorectal cancer was 65.71%( 46/70),which was significantly higher than that in normal intestinal mucosa( P< 0.001). The difference was significant between poorlydifferentiated and highly-mediumly differentiated in positive rate of HMGA2 in colorectal cancer( P<0.05). Positive rate of HMGA2 incolorectal cancer of TNM Ⅲ + Ⅳ stage was significantly higher than that of Ⅰ + Ⅱ stage( P<0.05).The incidence of MSI-H in color-ectal cancer was 17.14%( 12/70). The incidence of MSI-H in the right colon was 31.03%( 9/29),which was higher than that in theleft colon( 7.32%,3/41)( P<0.05).There was no significant difference in positive rate of HMGA2 between MSI-H group and MSI-L/MSS group( P> 0.05).Conclusions The high expression of HMGA2 may promote the occurrence and development of colorectal canc-er. The mechanism of action is independent of microsatellite status and needs further study.
