摘要
目的:分析10种常见恶性肿瘤住院费用的构成情况及影响因素。方法:选取某肿瘤医院2009-2013年10个病种共197593份病历资料进行分析;采用结构变动度、结构变动值等指标来分析费用的特征、分布及构成情况;采用递归系统模型分析各相关因素对住院费用的影响。结果:病种间费用结构差异大,药费占比高,技术性收费低;性别、年份、病种、是否首次住院、是否医保、是否手术对住院费用既有直接效应也有间接效应,而病人来源对住院费用只有直接效应。结论:合理控制住院天数、优化住院费用结构是控制住院费用的关键。
Objective: To study the hospitalization expense and its factors of ten common cancers. Methods: A total of 197593 medical records from 2009 to 2013 were extracted. We use the structural variation degree and structural change to perform statistical analysis, then use the recursive system model to analyze the impact of relevant factors on the hospital costs. Results: The cost structure among different diseases vary a lot. The proportion of drug cost was high and the items reflecting technology and skill is low. It shows that the indexes such as gender, year, diseases, whether admitted for the first time, whether under medical insurance, whether surgery not only have a direct impact, but also have indirect effects on the cost of hospitalization, while the indexes such as patients resource just have direct effects on the cost of hospitalization. Conclusion: Shortening the length of hospitalization and optimizing the structure of costs are the key points to reduce the hospitalization expense.
