中美医院感染诊断标准在某医院感染现患率调查中应用对比分析 被引量:43

摘要 目的了解中国和美国医院感染诊断标准差异,为有效防控医院感染提供诊断依据。方法采用中美两国医院感染诊断标准,对中国某医院住院患者医院感染现患率调查作对比分析。结果调查当日共有住院患者975例,依据中国诊断标准确诊感染患者23例,医院感染现患率为2. 4%,其中包括血管插管相关感染率为1. 8%,尿管相关性感染率为3. 3%,呼吸机相关性肺炎发生率为33. 3%。依据美国诊断标准确诊医院感染患者34例,医院感染现患率为3. 5%,其中包括血管插管相关感染率为3. 0%,尿管相关性感染率为3. 9%,呼吸机相关性肺炎发生率为41. 7%。2组医院感染率之间差异有统计学意义,器械相关性感染发生率之间差异无统计学意义。结论本研究跟据中美两国医院感染诊断标准所进行的横断面调查发现,医院感染现患率差异有统计学意义,器械相关性感染发生率差异无统计学意义。 Objective To analyze the diagnostic criteria of nosocomial infection in China and the United States,and to provide important basis for the control and prevention of nosocomial infection.Methods Using the diagnostic criteria of nosocomial infection between China and the United States,the prevalence of nosocomial infection among inpatients in a hospital in China was compared and analyzed.Results Among 975 inpatients from the hospital,using the diagnostic criteria of nosocomial infection in China,23 cases of hospital-infected patients were identified,with the prevalence rate of 2.4%.And the incidence for central line-associated bloodstream infection(CLBSI),catheter-associated urinary tract infection(CAUTI)and ventilator-associated pneumonia(VAP)were 1.8%,3.3%and 33.3%respectively.In the diagnostic criteria of the United States,34 cases of nosocomial infection were identified,and the prevalence rate was 3.5%.The incidence for CLABSI,CAUTI and VAP were 3.0%,3.9%and 41.7%respectively.There is statistical significance between the two groups in nosocomial infection rate,and there was no significant difference in the incidence of device-related infection between the two groups.Conclusions According to diagnostic criteria from China and the United States,there is statistical significance between the two groups in nosocomial infection rate.And no significant differences are found for the rates of device-associated infections.
