河南省猪源耐甲氧西林金黄色葡萄球菌的耐药性及分子分型研究 被引量:15

Antimicrobial resistance and molecular typing of methicillin-resistant Staphylococcus aureus from pigs in Henan Province

摘要 本研究旨在阐明河南省猪源耐甲氧西林金黄色葡萄球菌(MRSA)的流行现状及耐药表型特征。从河南省3个规模化养猪场和1个生猪屠宰场共采集1 107份猪鼻腔拭子,采用选择性培养基分离金黄色葡萄球菌与MRSA疑似株,运用多重PCR(扩增16SrRNA、nuc和mecA)鉴定金黄色葡萄球菌和MRSA;以纸片扩散法检测MRSA分离株对19种抗菌药物的敏感性;运用脉冲场凝胶电泳(PFGE)、多位点序列分型(MLST)和葡萄球菌蛋白A(spa)分型方法对MRSA分离株进行分子分型。结果显示,共检出358株金黄色葡萄球菌,分离率为32.3%,其中119株为MRSA,分离率为10.8%,金黄色葡萄球菌分离株中MRSA的阳性率为33.2%。有66.39%的MRSA分离株同时耐受15种抗菌药物,其中出现了喹奴普汀/达福普汀(1.7%)和利福平(0.8%)耐药株,但尚未出现万古霉素和利奈唑胺耐药株。PFGE结果显示,河南省猪群流行的MRSA遗传谱系较为复杂。MLST和spa分型结果显示,该地区猪群流行的MRSA主要为ST9-t899型。研究结果表明,河南省猪群MRSA广泛流行,多重耐药十分严重;流行的猪源MRSA主要为ST9-t899型,但遗传背景呈多样化。本研究结果为加强我国生猪养殖重点地区猪源MRSA的监测和流行传播机制研究奠定了基础。 The objective of the present study was to determine the prevalence and phenotypic profiles of methicillin-resistant Staphylococcus aureus(MRSA)from pigs in Henan provinces.Nasal samples(n=1 107)were collected from pigs on 3farms and 1slaughtering house.S.aureus and MRSA were isolated by using the selective medium and identified using multiplex PCR(16SrRNA and nuc for SA;16SrRNA,nuc and mecAfor MRSA).Antimicrobial susceptibility testing was performed using disk diffusion,and the colony relatedness of these MRSA strains were analyzed by the pulsed-field gel electrophoresis(PFGE),multilocus sequence typing(MLST)and staphylococcal protein A(spa)typing.A total of 358 S.aureusisolates,including 119 MRSA isolates,were identified from 1 107 pig nasal samples.66.39% of the MRSA isolates were simultaneously resistant to 15 antimicrobials,and the resistance rates of quinupristin/dalfoopristin and rifampin were 1.7% and 0.8%,respectively.However,no isolates were resistant to vancomycin and linezolid.The results of PFGE showed diverse genetic profiles of these MRSA isolates.The major ST type of these MRSA isolates from pigs was ST9,and the major spatype was t899.The isolating rate of MRSA from pigs in Henan Province was higher than that from other provinces in China and the MRSA isolates of diverse profiles were seriously resistant to most antimicrobial agents.The majority of MRSA isolates was ST9-t899 type.The findings of this study provided base-line data for monitoring the dissemination of MRSA and conducting effective control measures in provinces of swine production in China.
