摘要
目的分析慢性肾脏病患者病案中护理费用影响因素。方法选取某院2017年1月-2017年12月慢性肾脏病患者护理病案250份,其中男性157份,自拟患者基础信息调查表,内含患者基本信息包括姓名、性别、出生年月日、住院时间、治疗费用、总体费用、保险、住院次数等。结果 250份病案总护理费用平均为679. 58±7. 48元,中位数为647.59元,一级护理平均费用7569. 72±854. 27元,中位数为4452. 14元,最低花费是四级护理,平均费用51. 29±9. 47元,中位数为84. 57元。青年组公费医疗比例最高,显著高于中年组与老年组;老年组患者社会保险报销率最高,高于青年组与中年组。中年组自费率高于青年组与老年组。3组护理费用差异比较均具有统计学意义。慢性肾脏病患者护理费用与年龄、疾病分期、住院时间、支付方式有关,其中护理费用与年龄、疾病分期、住院时间呈正相关性。结论影响慢性肾脏病患者护理费用多少的因素较多,包括疾病分期、年龄、住院时间等,合理制定慢性肾脏病患者护理收费标准,提醒患者尽早住院治疗是减少护理费用的有效途径。
Objective To investigate the nursing expenses influencing factors in patients with chronic kidney disease.Methods 250 valid medical records of nursing record in our hospital from January 2017 to December 2017 were selected,including 157 cases of male patients.The data including the name,gender,and date of birth,hospital stay,treatment cost,overall cost,insurance,number of hospitalizations,etc.were collected through a self-made questionnaire.Results Among 250 cases of medical records,the median consumption of total nursing expenses was 647.59 yuan with an average of 679.58±7.48 yuan,the first-level median consumption was4452.14 yuan with an average of 7569.72±854.27 yuan,and the lowest-level median consumption was 84.57 yuan with an average of 51.29±9.47 yuan.The proportion of public medical expenses in the youth group was significantly higher than that in the middle-aged group and the elderly group,and the social insurance reimbursement rate was the highest in the elderly group,which was higher than that in the young group and the middle-aged group.There were statistically significant differences in nursing expenses between the three groups.Conclusion There are many factors affecting the nursing cost of patients with chronic kidney disease,including disease stage,age,length of stay,etc.,so it is an effective way to reasonably set the nursing fee standard for patients with chronic kidney disease and remind them to be hospitalized as soon as possible to reduce the nursing cost.
