摘要
目的分析神经内科眩晕病症患者临床疗效及其影响因素。方法 2016年1月-2018年1月高唐县人民医院神经内科共收治了眩晕为主要表现的患者156例。按照预后分为良好组41例、不良组115例。比较两组患者的临床表现并分析预后影响因素。结果不良组年龄、BMI、入院时NIHSS评分、眩晕发作最长持续时间高于良好组,差异有统计学意义(P<0.05)。不良组与良好组伴随症状、吸烟/饮酒、高血压、糖尿病、代谢紊乱、颈动脉严重狭窄、椎基底动脉严重狭窄率比重差异有统计学意义(P<0.05)。吸烟/饮酒、椎基底动脉严重狭窄、代谢紊乱成为独立影响因素,差异有统计学意义(P<0.05)。结论神经内科眩晕病症患者疗效存在较大个体差异,除常规治疗外,需要禁烟酒,积极治疗椎基底动脉严重狭窄,纠正代谢紊乱。
Objective To analyze clinical efficacy and influencing factors of patients with neurological vertigo.Methods From January 2016 to January 2018,156 patients with vertigo were treated in the Department of Neurology,Gaotang Country People’s Hospital.According to the prognosis,the patients were divided into good group with 41 cases and bad group with 115 cases.The clinical manifestations of the two groups were compared and the prognostic factors were analyzed.Results The age,BMI,NIHSS score at admission and the longest duration of vertigo attack in the bad group were higher than those in the good group,the differences were statistically significant(P<0.05).There were significant differences in the simultaneous phenomenon,smoking/drinking,hypertension,diabetes mellitus,metabolic disorders,severe stenosis of carotid artery and severe stenosis of vertebrobasilar artery between the bad group and the good group(P<0.05).Smoking/drinking,severe stenosis of vertebrobasilar artery and metabolic disorder were independent influencing factors,the differences were statistically significant(P<0.05).Conclusion There are great individual differences in the efficacy of neurological vertigo patients.In addition to routine treatment,attention should be paid to smoking and alcohol prohibition,active treatment of severe vertebral basilar artery stenosis,and correction of metabolic disorders.
