摘要
目的探讨家庭访视干预对老年高血压患者服药依从性的影响。方法选取自贡市某社区卫生服务站的80例老年高血压患者为研究对象,采用随机数字法将其随机分为两组,每组40例,对照组患者采用社区高血压疾病常规管理,观察组患者在对照组的基础上进行家庭访视干预。干预6个月后,比较两组患者服药依从性及血压变化情况。结果干预后,观察组患者适应度、合作度、成长度、亲密度等家庭功能评分明显优于干预前及对照组,差异有统计学意义(P<0.05)。干预后,观察组患者自行停止服药、自行减少服药次数、自行减少服药剂量、按医嘱定时服用药物、按医嘱定量服用药物等行为评分明显高于对照组,组间比较差异有统计学意义(P<0.05)。干预后,两组患者舒张压、收缩压较干预前发生不同程度的降低,其中观察组患者下降幅度明显高于对照组,组间比较差异有统计学意义(P<0.05)。干预后,观察组患者健康行为评分、疾病自我行为报告评分以及自我管理能力评分均明显高于干预前及对照组,且差异有统计学意义(P<0.05)。结论家庭访视干预可有效提高老年高血压患者家庭功能、健康行为能力以及自我管理能力,并通过改善老年高血压患者服药依从性达到有效控制患者血压的效果。
Objective To investigate the effect of family visit intervention on medication adherence behaviors in elderly patients with hypertension.Methods Totally 80 elderly patients with hypertension were recruited from a community health service station of Zigong city.They were randomly divided into two groups,40 patients in each group.The control group was treated by routine management,meanwhile the observation group was treated by routine management coupled with family visit intervention.After six months,the drug compliance and blood pressure changes of these two groups were compared.Results After intervention,the fitness,cooperation,growth and closeness of the observation group were significantly better than the control group,and the difference was statistically significant(P<0.05).The score of stop taking,reducing frequency of medication,reducing dose,takeing medicine regularly and quantitatively as directed in observition group were significantly higher than the control group(P<0.05).After intervention,the diastolic blood pressure and systolic blood pressure in the two groups were both discreased,and there were significant differences between the two groups(P<0.05).After intervention,the patients’health behavior score,disease self-behavior report score and self-management ability score in the observation group were significantly higher than those in the control group(P<0.05).Conclusion Family visit intervention can effectively improve the elderly patients with hypertension in family function,health behavior ability and self management ability.Effect of blood pressure control were enhanced by improving the drug compliance of the elderly patients with hypertension.
