出血性与缺血性脑卒中住院患者危险因素比较分析 被引量:3

The research on the risk factors' distribution about the hospitalized patients with ischemic and hemorrhagic stroke

摘要 目的比较分析出血性与缺血性脑卒中患者的危险因素,为进一步预防控制脑卒中提供依据。方法从福建医科大学附属第一医院神经内科脑卒中数据库中,收集2013年1月1日~12月31日连续入院的脑卒中患者393例,其中出血性脑卒中65例,缺血性脑卒中328例,收集相关的危险因素。采用SPSS 20.0统计软件进行数据分析,组间危险因素比较采用χ2检验,并采用二分类logistic回归分析进行筛选,P<0.05为差异有统计学意义。结果单因素分析结果显示,出血性和缺血性脑卒中患者年龄、高血压、糖尿病、心房纤颤和颈动脉粥样硬化的构成比差异有统计学意义(P<0.05)。二分类logistic回归分析显示,≥65岁、糖尿病、颈动脉粥样硬化、心房纤颤患者更易发生缺血性脑卒中,高血压患者更易发生出血性脑卒中。结论不同脑卒中亚型危险因素的重要性不同,有针对性地加强防治,对降低脑卒中的发生率和病死率具有重要意义。 Objective To compare the risk factors for ischemic and hemorrhagic stroke in order to provide the basis for further control and prevention of stroke. Methods A total of 393 patients with stroke(65 cases with hemorrhagic stroke and the rest with ischemic stroke) were collected from the database of the Department of Neurology, the First Affiliated Hospital of Fujian Medical University between January 1 and December 31 in 2013. A comparison of the common risk factors of ischemic and hemorrhagic stroke was performed. Risk factors between the two groups were tested by χ2test and were screened by logistic regression analysis with SPSS 20.0 and P<0.05 was considered statistically significant. Results The results using single factor analysis showed that the differences of age, hypertension, diabetes, atrial fibrillation and carotid atherosclerosis in hemorrhagic and ischemic stroke were statistically significant(P<0.05). Binary logistic regression analysis showed that the risk factors of 65 years old and above, diabetes, carotid atherosclerosis, atrial fibrillation were more likely to lead to ischemic stroke. Hypertension was more likely to lead to hemorrhagic stroke. Conclusions Different stroke subtypes have different important risk factors. Therefore it is important to reduce the incidence and mortality of stroke by making a risk factor-targeted approach to improve prevention and control.
