经腹部二维超声与三维超声观察胎儿颅脑中线结构的对比

摘要 目的:研究对比经腹部二维超声与三维超声容积对比成像联合C平面(VIC-C)技术获得的胎儿头颅正中矢状切面图像,为诊断胎儿颅脑中线结构畸形时选取何种扫查方法提供一定的依据。方法:对149例19~34周正常胎儿,分别经腹部二维超声与三维超声(VIC-C)扫查胎儿头颅获得正中矢状切面(三维超声选取小脑横径平面作为成像始基平面),统计两种扫查方法颅脑正中矢状切面的获得率,正中矢状切面上胼胝体、小脑蚓部等重要中线结构的显示率,分别进行比较及统计学分析。结果:二维获得率39.6%,三维获得率90.6%,P <0.05,差异具有统计学意义。分别统计两种扫查方法可获得正中矢状切面的病例中,胼胝体显示率分别为93.2%、94.8%,透明隔腔显示率分别为94.9%、95.6%,小脑蚓部显示率分别为96.6%、96.2%,第四脑室显示率分别为72.8%、70.4%,均P> 0.05,差异不具有统计学意义。结论:二维、三维超声获得的正中矢状切面图像质量差异不大,而三维超声可以更快速可行的获得胎儿颅脑正中矢状切面,可有效提高诊断中线结构异常的效率。 Objective To compare the image of sagittal segment of fetal skull by two-dimensional and three-dimensional(VCI-C technology) ultrasound of abdomen,To provide some basis for the diagnosis of fetal craniocerebral structural malformations. Methods For 149 cases of 19 ~ 34 weeks of normal foetuses,The mid-sagittal section was obtained by scanning the fetal head with two-dimensional and three-dimensional ultrasound(VIC-C technology), Three-dimensional ultrasound selects the transverse diameter plane of the cerebellum as the initial base plane of imaging.Statistics on the acquisition of sagittal segment in the middle of the brain by two methods.The display rate of important midline structures such as corpus callosum and cerebellum on The central sagittal section.Comparison and statistical analysis,respectively.Results Two-dimensional ultrasound dispaly rate was 39.6%, Three-dimensional ultrasound median dispaly rate was 90.6%,P<0.05,The difference is statistically significant.Statistics on the cases of the middle sagittal section obtained by the two sweeping methods,The corpus callosum display rate was 93.2% and 94.8%,The transparent compartment display rate was 94.9 % and 95.6 %,The display rate of cerebellar vermis was96.6%、96.2%,The display rate of fourth ventricle was72.8% and 70.4%. P>0.05,The difference is not statistically significant.Conclusion The image quality of the central sagittal segment obtained by two-dimensional and three-dimensional ultrasound is not very different. Three-dimensional ultrasound can obtain the sagittal section of the fetal brain more quickly and effectively.It can effectively improve the efficiency of diagnosis of medial structural anomaly.
