摘要
目的分析虎红平板凝集试验(Rose-Bengal Plate Test,RBT)、试管凝集试验(Standard-tube Agglutination Test,SAT)和酶联免疫吸附试验(Enzyme-Linked Immuno Sorbent Assay,ELISA)在布鲁氏菌病(简称布病)中的检测结果,客观评价3种方法在布病检测中的意义。方法同时采用虎红平板凝集试验(RBT)、试管凝集试验(SAT)及酶联免疫吸附试验(ELISA:IgG,IgM) 3种方法对内蒙古自治区综合疾病预防控制中心门诊部收集的1 000份样本进行血清学检测并比较分析。结果 3种方法的检测结果为:RBT阳性169份,SAT阳性143份,IgG阳性345份,IgM阳性361份。以SAT作为布病诊断的金标准对其他3种方法进行对比分析,SAT和RBPT差异有统计学意义(χ~2=19.53,P <0.05);SAT和IgG差异有统计学意义(χ~2=159.39,P <0.05);SAT和IgM差异有统计学意义(χ~2=160.55,P <0.05)。RBT与SAT相比:灵敏度97.90%(140/143)、特异度96.62%(828/857)。IgG和SAT相比:灵敏度81.12%(116/143)、特异度73.28%(628/857)。IgM和SAT相比:灵敏度72.73%(104/143)、特异度70.01%(600/857)。结论RBT的灵敏度和特异度均在95%以上,IgG有相对较高的灵敏度和特异度,灵敏度也高于IgM。因此,RBT适合于布病的大面积筛查,再使用较稳定的SAT进行确诊,这对布病的早期诊断有重要的意义。
Objective To analyze the results of rose bengal precipitation test(RBT),standard tube agglutination test(SAT)and enzyme-Linked immuno Sorbent assay(ELISA)in brucellosis and objectively evaluate the significance of three methods in brucellosis detection.Methods Meanwhile,bengal precipitation test,standard tube agglutination test and enzyme-linked immunosorbent assay were used to detect and compare the serology of 1 000 samples collected by the outpatient department of the comprehensive center for disease control and prevention of Inner Mongolia autonomous region.Results The results of the three methods were as follows:RBT positive serum 169 cases,SAT positive serum 143 cases,IGG positive serum 345 cases;IGM positive serum 361 cases.The SAT was used as the gold standard for the diagnosis of brucellosis,and the difference between the SAT and RBT was statistically significant(χ~2=19.53,P<0.05).The difference between SAT and IgG was statistically significant(χ~2=159.39,P<0.05).The difference between SAT and IgM was statistically significant(χ~2=160.55,P<0.05).Compared with SAT,BPT has a sensitivity of 97.90%(140/143)and a specificity of 96.62%(828/857).Compared with SAT,IgG had a sensitivity of 81.12%(116/143)and a specificity of 73.28%(628/857).Compared with SAT,IGM had a sensitivity of72.73%(104/143)and a specificity of 70.01%(600/857).Conclusion The sensitivity and specificity of RBT were above95%,IgG had relatively high sensitivity and specificity,and the sensitivity was also higher than IgM.Therefore,RBT is suitable for extensive screening of brucellosis,and then stable SAT is used for diagnosis,which is of great significance for the early diagnosis of brucellosis.
