摘要
目的:探讨集束化护理对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)气管插管患者呼吸机相关性肺炎(ventilatorassociated pneumonia,VAP)的影响。方法:选取笔者所在医院呼吸内科于2018年8月-2019年2月收治的COPD气管插管患者100例,采用随机数字表法将其分为对照组和试验组,每组50例。对照组采用常规护理,试验组在常规护理基础上采用集束化模式护理。比较两组患者呼吸机相关性肺炎发生率及血气分析相关指标、机械通气时间、总住院时间及护理满意度。结果:试验组VAP发生率为18.0%,明显低于对照组的62.0%,差异有统计学意义(P<0.05);试验组血气分析相关指标改善情况均明显优于对照组,差异有统计学意义(P<0.05);试验组机械通气时间、总住院时间均短于对照组,差异有统计学意义(P<0.05);试验组患者护理满意度评分高于对照组,差异有统计学意义(P<0.05)。结论:集束化护理模式能有效预防VAP的发生,改善患者血气分析各项指标,缩短住院时间,促进患者康复,改善护患关系。
Objective:To investigate the effect of cluster nursing on ventilator-associated pneumonia(VAP)in patients with chronic obstructive pulmonary disease(COPD)tracheal intubation.Method:A total of 100 patients with COPD tracheal intubation admitted in the department of respiratory medicine of our hospital from August 2018 to February 2019 were selected.They were randomly divided into control group and experimental group by random number table method,50 cases in each group.The control group was treated with routine nursing.The experimental group was treated with cluster nursing on the basis of routine nursing.The incidence of ventilator-associated pneumonia,blood gas analysis related indicators,mechanical ventilation time,total hospitalization time and nursing satisfaction were compared between the two groups.Result:The incidence of VAP in the experimental group was 18.0%,which was significantly lower than 62.0%in the control group,and the difference was statistically significant(P<0.05).The improvement of blood gas analysis related indicators in the experimental group were significantly better than those in the control group,the differences were statistically significant(P<0.05).The mechanical ventilation time and total hospitalization time in the experimental group were shorter than those in the control group,the differences were statistically significant(P<0.05).The total satisfaction score of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:The cluster nursing mode can effectively prevent the occurrence of VAP,improve the blood gas analysis indicators of patients,shorten the hospitalization time,and promote the rehabilitation of patients,and improve the nurse-patient relationship.
