摘要
目的:研究某ICU病原学分布,为临床合理用药提供依据。方法:收集2014年1月-2018年12月入住ICU体液培养阳性病例,回顾性分析ICU常见病原菌,通过SPSS 11.0软件分析2014-2018年ICU病原学特点,分析身体不同部位感染病原学特点。结果:共收集2 866份培养阳性病例,其中前10位检出菌为鲍曼不动杆菌432株(15.1%),铜绿假单胞菌405株(14.1%),肺炎克雷白杆菌384株(13.4%),大肠埃希菌329株(11.5%),肠球菌247株(8.6%),金黄色葡萄球菌130株(4.5%),嗜麦芽窄食单胞菌121株(4.2%),洋葱伯克霍尔德菌120株(4.2%),白假丝酵母菌85株(3.0%),阴沟肠杆菌54株(2.0%),其他559株(19.5%)。2014-2018年ICU的病原菌分布以革兰阴性杆菌为主。各部位检出常见病原菌有所不同,痰及肺泡灌洗液检出病原菌1 152株,最常见鲍曼不动杆菌286株(24.8%);尿液检出病原菌298株,最常见大肠埃希菌61株(20.5%),腹水及胆汁检出病原菌400株,最常见大肠杆菌78株(19.5%);胸水检出病原菌118株,最常见肺炎克雷白杆菌18株(15.2%);皮肤伤口分泌物检出病原菌172株,最常见鲍曼不动杆菌、肺炎克雷白杆菌、大肠埃希菌各22株(各占12.8%),血培养阳性335株,最常见肺炎克雷白杆菌64株(19.1%)。结论:ICU常见病原菌为肠杆菌科细菌,其中各个部位病原学检测结果有区别,可根据感染的不同部位选取不同抗感染药物。
Objective:To study the distribution of etiology in ICU and provide evidence for clinical rational drug use.Method:Results of fluid culture of patients admitted to ICU from January 2014 to December 2018 were collected.Retrospective analysis of common pathogenic bacteria in ICU.The pathogenic changes from 2014 to 2018 were analyzed by SPSS 11.0 software,and the pathogenic characteristics of infections in different parts of the body were analyzed.Result:A total of 2 866 specimens were collected.The top 10 bacteria were Acinetobacter Baumannii 432 case(15.1%),Pseudomonas Aeruginosa 405 cases(14.1%),Klebsiella Pneumoniae 384 cases(13.4%),Escherichia Coli 329 cases(11.5%),Enterococcus 247 cases(8.6%),Staphylococcus Aureus 130 cases(4.5%),Stenotrophomonas Maltophilia 121 cases(4.2%),Burkholderia Cepacia 120 cases(4.2%),Candida Albicans 85 cases(3.0%),Enterobacter Cloacae 54 cases(2.0%),other 559 cases(19.5%).From 2014 to 2018,the distribution of pathogenic bacteria in ICU was dominated by gram negative bacilli.The first common pathogenic bacteria,phlegm and bronchoalveolar lavage fluid detected pathogens in 1 152 cases,the most common Acinetobacter Baumannii in 286 cases(24.8%).Urine detection pathogens in 298 cases,the most common Escherichia Coli 61 cases(20.5%),ascites and bile detected pathogens in 400 cases,the most common Escherichia Coli 78 cases(19.5%).Pleural effusion detected pathogens in 118 cases,the most common Klebsiella Pneumonia 18 cases(15.2%).Skin wound secretion detected pathogens in 172 cases,there were 22 cases of Acinetobacter Baumannii,Klebsiella Pneumoniae and Escherichia Coli(each accounting for 12.8%).335 cases of positive blood culture,and 64 cases of Klebsiella Pneumoniae(19.1%).Conclusion:The common pathogenic bacteria in ICU are enterobacteriaceae,and the results of pathogen detection in different parts of the ICU are different.Different anti-infective drugs can be selected according to different parts of the infection.
