摘要
目的:探讨内侧撑开胫骨高位截骨矫正膝内翻术后胫骨轴向旋转改变的影响因素。方法:利用CT扫描数据建立胫腓骨近端的三维几何模型,在此模型基础上通过几何操作,模拟以内侧撑开方式进行胫骨高位截骨矫正膝关节内翻,分析截骨矫正过程中合页轴的方向及内翻矫正程度与术后胫骨轴向旋转之间的关系。结果:以矢状位前后方向作为参考,与此相比,当合页轴的方向指向前外侧时会导致术后胫骨内旋增大;同时,需要矫正的内翻角度越大,术后胫骨内旋增加越多。结论:内侧撑开胫骨高位截骨术后胫骨轴向旋转的变化与截骨矫正过程中合页轴的方向及内翻矫正程度密切相关。
Objective To explore factors affecting axial rotation of tibia after the medial open-wedge high tibial osteotomy.Methods The simulated medial open-wedge high tibial osteotomy was performed on the three-dimensional models based on computed tomography(CT)scanning,and the roles of hinge axis direction and correction of genu varum in postoperative axial rotation of tibia were analyzed respectively.Results The hinge axis pointing anterolaterally was significantly associated with the increase of the postoperative internal rotation of the tibia with regard to the axis along the anterior-posterior direction.Meanwhile,larger correction of genu varum was also a crucial factor for the increase of postoperative internal rotation of tibia.Conclusion Postoperative axial rotation of tibia after medial open-wedge high tibial osteotomy is closely associated with the hinge axis direction and the correction of genu varum.
