摘要
目的 :评价磁共振弥散加权成像对急性脑梗塞的诊断价值及对急性和非急性脑梗塞的鉴别诊断价值。方法 :对临床怀疑急性脑中风 35例患者 ,进行常规T2 WI和DWI检查 ,并采用盲法进行对照观察。对DWI图像上出现的异常高信号的病变部位进行准确定位 ,并判断病变部位与临床症状与体征是否相符 ,对DWI图像上出现的异常高信号与FSE序列T2 WI像上的表现进行对照观察。结果 :35例在DWI像上均出现异常信号 ,其中 3例是在发病后 6h内进行MRI检查 ,而在常规T2 WI像上未显示异常信号 ;2例是在发病后 12h内进行检查 ,而在常规T2 WI像上仅表现稍高信号 ;10例患者在发病后 12~ 2 4h内进行检查 ,2 0例患者在发病后 2 4~ 72h进行检查 ,在T2 WI和DWI像上均出现异常高信号 ;其中 8例在FSE T2 WI像上表现为多发脑梗塞病灶 ,而在DWI像上显示为单一异常高信号。在DWI像上诊断急性脑梗塞的敏感性及特异性均为 10 0 %。结论 :DWI图像比常规T2 WI图像对诊断急性脑梗塞有较高的敏感性及特异性 ,能发现常规T2 WI像不能发现的超早期脑梗塞 ,并能鉴别急性和非急性脑梗塞 ,故DWI图像在诊断急性脑梗塞具有十分重要的诊断价值。
Objective:To appraise the value of diffusion weighted imaging in diagnosis of acute cerebral infarction and the efficacy in differantiate acute from non acute cerebral infarction.MethodsThirty five patients with clinically diagnosed acute cerebral infarction were scanned with both conventional T 2WI and DWI.All the images were double bindly reviewed and compared.The abnormal high signal area of lesions seen on DWI were precisely located,and judged whether the avea of lesion seen in DWI was in agreement with the patient's symptoms and signs.The abnormal high signal seen on DWI was contrasted and observed with the images of FSE T 2WI.Results:Thirty five patients with clinically diagnosed acute cerebral infarction all showed abnormal high signal on DWI.Of the thirty five patients,three patients were scanned with MRI within six hours after the lesion came on,but the abnormal signal did not show on the conventional T 2WI.Tow patients were scanned within twelve hours after the lesion came on ,but it only showed slightly high signal in FSE T 2WI.Ten patients were scanned within twelve to twenty four hours after the lesions came on,the abnormal high signal demonstrated on both T 2WI and DWI.Twenty patients were scanned within twenty four to Seventy two hours after the lesion came on,and in eight patients FSE T 2WI demonstrated as multiple cerebral infarctions,which were proved to have only one abnormal high signal on DWI,The sensitivity and specificity of DWI in diagnosing acute cerebral infraction were both 100%Conclusion:[WT5”BZ]DWI is of higher sensitivity and specificity compared with T 2WI for diagnosing acute cerebral infarction and detecting early infarction lesions,which are difficult to be displayed on conventional T 2WI,and it can also differentiate acute from non acute cerebral infraction.DWI is of high accuracy and value in diagnosing acute cerebral infarction.
