摘要
目的 :对恒牙期安氏Ⅱ1类下颌后缩患者 ,以Herbst矫治器矫治并排齐牙列后的效果进行观察。方法 :用Herbst矫治器矫治 15名 12～ 15岁恒牙期Ⅱ1类下颌后缩患者磨牙达中性偏近中关系后 ,以Edgewise进一步矫治使后牙达到完善的牙合接触关系 ;以X线头影测量方法评价疗效。结果 :ANB角减小了 1.2 9° ;前牙覆盖减小了 5 .37mm,骨性变化占 5 3%、牙性变化占 4 7% ,磨牙关系改善了 4 .97mm ,骨性变化占 5 7%、牙性变化占 4 3% ,其中下颌骨前移了 3.6 0mm对两者的矫正占了主要作用 ,上颌骨向前生长了 0 .78mm ;有效髁突向上生长了 4 .4 8mm ,向后改变了 3.4 1mm ;下颌平面角无明显变化。结论 :Herbst矫治器对恒牙期安氏Ⅱ1类下颌后缩的矫治具有显著的疗效 。
Objective:To investigate the effects induced by Herbst therapy of Class Ⅱdivision 1.Methods:15 subjects with a ClassⅡ Division 1 malocclusion were treated during the skeletal maturity stage MP3-FG and -G with the Herbst appliance followed by comprehensive Edgewise therapy.The cephalometric analysis were used to evaluate the effects.Results:SNB increased 1.29°.Overjet correction averaged 5.37 mm was due to 53% skeletal and 47% dental changes. ClassⅡmolar correction averaged 4.97 mm was due to 57% skeletal and 43% dental changes.The mandibular advanced 3.6mm,while the maxillary has a limited anterior movement of 0.78mm.The effective condylar growth was upward 4.48mm and backward 3.41mm.Conclusions:The achieved occlusal correction was resulted from skeletal and partly dental changes,but skeletal effects were due mainly to mandibular changes.
